2344354 Primary Facility 2015-02-04 Routine/Complete Inspection No Violations Noted H&P Rig #284 is the active drilling rig on the location. I met with Jacob Oettel, company man, of Warren E&P. Mr. Oettel stated that drilling procedures for the Mirabelli Unit 3H well have been just recently completed. The well has been drilled to total depth, which is around (12,800) feet. The Mirabelli 3H and the Mirabelli Keystone 1H wells have been drilled to a depth of (2,032) feet. The surface and intermediate casing has been cemented on each of these wells. It is not clear as to when Warren will return to the location to drill the wells to total depth.

2849666 Primary Facility 2019-02-04 Drilling/Alteration No Violations Noted I arrived at the site at 12:15. I was accompanied by Casey Baldwin, DEP O&G WQS out of the Scranton District Office. The weather conditions are partly cloudy skies, light winds, and mild with temperatures in the low to mid 50's. Precision Drilling Rig #532 is on the location. BKV submitted a Drilling Resumption After Break of 30 days or more Notification to the Department on 1/29/19. The notice indicated BKV would resume drillng of the Keystone Unit Mirabelli 1 Keystone 1H well at 6AM on 2/1/19. The surface and intermediate sections of this well, and the Keystone Unit Mirabelli 1-3H were drilled and cemented in 2015.

We met Kevin Brady, Hydrologist, Environmental Scientist, Wetland Delineator (WPIT) of Whitetail Environmental Services, LLC, Gary Titus, site company man, consultant of BKV, Larry Gerard, Completions Superintendent, and Gary King, Drilling Superintendent, both consultants of BKV. The current drilling depth on the Mirabelli Keystone 1H well is (6,102) feet. BKV provided the following d
