2713319 Primary Facility 2018-03-28 Routine/Complete Inspection No Violations Noted Arrived at site at: 08:30. Weather: Overcast, cold ~ 35 degs. Onsite to perform routine inspections of wells on pad. A total of (9) wells were present on pad. All (9) wells were in production at time of inspection. All wells Prod. tree top press. "PTTP", Prod. line press. "PLP" and Prod. casing press. "PCP" were being monitored by wireless data sending units and/or analog gauges. (See individual well pressures and observations listed below.)

- All wells had (2) annular vents present labeled (13 3/8"x 9 5/8") and (9 5/8"x 5 1/2"). All annular vents were open and venting to (3) poly vent tanks "VTs" via shared 2" vent lines. I labeled the VTs with a black magic marker at time of inspection. (#1 VT shares the 2H, 5H, 3H & N1H annular vents. #2 VT shares the N4H, N2H & N3H annular vents. #3 VT shares the 1H & 4H annular vents.) I performed gas checks at VT lid openings using a Altair 5X gas meter calibrated on 3/5/18. Gas checks showed: VT #1 = 48% LEL, 2% CH4; VT #2 = 10% LEL, 0% CH4; VT #3 = 40% LEL, 2% CH4.

2743661 Primary Facility 2018-06-12 Routine/Complete Inspection No Violations Noted I conducted the inspection accompanied by Jon Sassi (EPS) & Casey Baldwin (WQS/ET). Upon arrival, we were approached by two unnamed Chief employees. We explained our purpose. After some discussion, we could ascertain that “pump-down” operations were being conducted at the site. Haliburton Oilfield Services were conducting the “pump-down” activity while wells were being hydraulically fractured at the nearby (approximately 1.93 miles) Cabot T. Flowers Well Pad. The "pump down" was occurring on the 1H well. There were no violations noted. If you have any questions, please contact me.
2804623 Primary Facility 2018-11-14 Routine/Complete Inspection No Violations Noted Chief Oil & Gas submitted a Notice of Termination (NOT) for the Garrison pad covered under ESX29-131-17-0003. The NOT was received by the Department on 9/27/18. Arrived onsite for at 10:35; the weather was ~30°F and calm. Several frozen puddles were on the pad surface.

There were 9 properly tagged Marcellus wells staged for production. Most of the well cellars were filled with water; no bubbling was observed. There were 9 GPUs and 1 compressor onsite and in secondary containment. There was approx. 6 to 12 inches of mostly frozen water in the produced water tank secondary containment. Secondary containment also remained around the wells. I contacted Chris Vassallo via email and was informed on 11/16/18 that the containment is there due to scheduled maintenance on one of the wells.

The site was well vegetated and appeared stable. All PCSM had been constructed and no temporary E&S BMPs remained.
