2760218 Primary Facility 2018-07-27 Routine/Complete Inspection No Violations Noted On July 27, 2018 Department representative Sue Herzing conducted a routine site inspection at COP TRACT 290 PAD A. The site sign, emergency response plan, monthly tank maintenance log, and weekly E&S inspection log were onsite for review. No E&S issues were observed during the time of the inspection. A PCSM infiltration berm has been installed per the PCSM drawings. This berm consists of 24" seeded compost sock. Vegetative growth on the sock is approximately 70%. The well pad remains its original footprint size. Overall, the site is stable and well vegetated exceeding 70% uniform, perennial, vegetative cover.

There are a total of 5 producing wells located on this well pad. All wells are tagged with API/well permit number. All well cellars are full of water. No bubbling within the cellars was observed. There are 5 GPU's and (4) 400 bbl steel vertical production fluids tanks onsite. Both GPUs and production fluids tanks are stationed in secondary containment. Water levels within the produced fluids containmen
