Lundy N 1H: tubing: 1292 psi, casing: 1066 psi Lundy N 2H: tubing 1301 psi, casing: 1215 psi Lundy N 3H: tubing 1214 psi, casing: 1129 psi.* well tag lists permit number as:081-20817. Should be 081-20965. Lundy N 4H: tubing 1251 psi, casing: 1129 psi. Lundy N 5H: tubing 1276 psi, casing: 1066 psi The status of permit # 081-20817 is, Proposed But Never Materialized. The Department recommends Atlas change the permit number on the Lundy N 3H well. Left location @ 12:30

The PCSM (Post Construction Stormwater Management) plan has been implemented as permitted and the stormwater features are functioning properly. The well site is considered restored and the appropriate WSRR's (Well Site Restoration Report) have been submitted.

CID# 321795 prompted the Department to request (in part) stormwater PCSM calculations which account for the underdrain. The operator satisfied our engineer's requirements to re-evaluate the site and have reduced the size of the orifice on the riser/outlet structure from 3" to 3/4". Based on this inspection, CID# 321795 is closed. The complainant was notified on 10/24/2016 that the complaint would be closed when the orifice size was reduced. No Violations Noted.

2782529 Primary Facility 2018-10-02 Routine/Complete Inspection No Violations Noted Five wells staged for production phase. Five each water separators. Two sand separators. No leaks detected from production equipment. Methanol tanks located near well heads. No violations noted.
2809784 Primary Facility 2018-12-04 Routine/Complete Inspection No Violations Noted Weather was 34° and cloudy at time of inspection. Met with Diversified Oil and Gas representative John Moltz on location at time of inspection. Well had digital pressure monitoring equipment attached to the production casing and the production tubing. Production casing was 1131 psi and production tubing was 1129 psi at time of inspection. There were 3 annulus vents plumbed to surface at time of inspection. All 3 vents were plumbed together and ran to the production tanks. No violations were noted at time of inspection.
