I entered the site at approximately 10:40 and talked to Tom Chitwood (SWN). The first 100+ feet of the access road and construction entrance was constructed and extra base rock was stockpiled for maintenance. The contractor used the existing road (withbase rock upgrades as needed) to get back to the last few hundred feet of access road and the well pad itself. The well pad area has been cleared and perimeter E&S controls have been installed. At the time of the inspection, the contractor was scraping topsoil from the shallow bedrock. The rock on the pad will be drilled and shot and the resulting material will be used as base rock throughout the site. Overall, the site is in good condition. No Violations Noted.

2601954 Primary Facility 2017-06-06 Routine/Complete Inspection No Violations Noted Routine inspection; 6/6/2017.

I entered the site at 13:18. At the time of the inspection, the majority of the bulk earthmoving was complete and work to establish drainage and ditches was underway. The topsoil stockpile was seeded and stabilized while disturbed soils along the access road were temporarily stabilized. Perimeter E&S controls are functioning properly and are in good condition no E&S issues were noted. Final grading and seeding/stabilization has yet to be completed. Top rock on the entire location will need to be completed as well. Overall the site looks good. No violations noted.

2608321 Primary Facility 2017-06-27 Routine/Complete Inspection No Violations Noted Routine inspection; 6/27/2017.

I entered the site at 08:55. At the time of the inspection, the access road was complete (including top-rock) and previously disturbed soils were seeded and stabilized. The berm for the infiltration basin was constructed and topsoiled. The under-drain, rock, geo-fabric, and amended soils were being installed on the infiltration basin. The pad area was complete except for clean-up, topsoiling, seed/stabilization, and top-rock. The E&S controls are installed and functioning properly. No E&S issues were noted. This inspection fulfills the pre-operational inspection requirement of the Oil and Gas Act as amended by Act 13. Overall the site looks good. No violations noted.

2638766 Primary Facility 2017-09-18 Drilling/Alteration No Violations Noted The site is well vegetated but not yet permanently stabilized. Pipeline is being tied-in on the northern edge of the pad while rig mats and barricades are staged onsite. The site is in good condition. No violations noted.
2665087 Primary Facility 2017-11-28 Routine/Complete Inspection No Violations Noted I met Jonathan Woodburn when I arrived. SWN rig 45 on location drilling the 3H well at a depth of 6793', currently drilling on air. Woody walked the site with me. Sitewide containment appeared to be in good condition, and secondary containments around the mud tanks and diesel tank were also in good condition. The site was clean and well maintained.
2669250 Primary Facility 2017-12-11 Routine/Complete Inspection No Violations Noted Weather was 28 degrees F. and snowing at time of inspection. Met with SWN representative Andrew Griffith. At time of inspection 13 3/8 casing was run into the hole to TD and cemented. Rig was experiencing difficulties performing a casing pressure test. Rigs Slip over/ Test plug surface equipment is was is determined to be leaking at time of inspection.mAndrew was in the process of trying to find a packer for 13 3/8 casing to perform a pressure test. No fresh water was detected during the drilling of then surface hole. Reviewed job log for surface casing cement job. Cement density and pressure remained consistent through out the cement job. 20 bbls of cement were returned to surface. Once rig can perform a successful pressure test on the 13 3/8 casing they will then proceed to drill the hole for the 9 5/8 intermediate casing. Approximate depth of the intermediate casing will be 1991'. No violations were noted at time of inspection.
2672542 Primary Facility 2017-12-15 Routine/Complete Inspection No Violations Noted Weather was 12 degrees F. and cloudy at time of inspection. Met with SWN representative. Andrew Griffith on location. Schlumberger was on location coming out of the hole from logging the 12 1/4 hole for the intermediate casing. Hole depth is 1991'. No fresh water shows were logged or encountered during drilling of intermediate hole. Rig was preparing to run the 9 5/8 casing. Cement job is anticipated to occur at approximately 4pm. No violations were noted at time of inspection.
2672593 Primary Facility 2017-12-19 Routine/Complete Inspection No Violations Noted Well it was 42° and partly sunny a time inspection. Met with SWN representative Andrew Griffith. Discussed cement job for the 9 5/8 intermediate casing. Casing was cemented on 12/15/17. Rig is currently drilling at 4784 feet and will continue drilling to 8 3/4" hole to anywhere between 5900' to 6070'. Kick off point is estimated to be at 5600 ft. They will then drill 8 1/2 hole to an estimated depth of 16371 ft. Location appears to be clean with good housekeeping measures in place. No violations noted at time of inspection.

2692545 Primary Facility 2018-02-06 Routine/Complete Inspection No Violations Noted When I was 26° and cloudy at time of inspection. Well was shut in. Gas production units were in the process of being installed on the well pad at time of inspection. Well has one annulus vent plumbed to surface from the 5 1/2 x 9 5/8 annulus. It was shut in at time of inspection. The 9 5/8 x 13 3/8 annulus and 13 3/8 x 20 annulus were visually observed to have cement to surface at time of inspection. There was an analog pressure gauge attached to the well head that would show the production casing pressure. At time of inspection the gauge was shut off by a needle valve. No violations were noted at time of inspection.
2699546 Primary Facility 2018-02-28 Routine/Complete Inspection No Violations Noted I met Mark MacPherson, company man, when I arrived. Frac was active, currently fracing stage 26 on the 3H well. Tracy Gregory of SWN arrived as I began my inspection, and accompanied me. Containment appeared to be in good condition. Nearly all of the puddles on the pad surface showed low conductance (.45-.65 mS/cm). There were 2 puddles that showed slightly higher conductance (1.0 mS/cm), one near the sandboxes (approximately 1 gallon) and one near the command trailer (5-10 gallons). Tracy informed Mac and instructed him to have these areas vacced.
2756407 Primary Facility 2018-07-26 Routine/Complete Inspection No Violations Noted Weather was 71° and sunny at time of inspection. There had been substantial rain over the last four days previous to inspection. There were no SWN representatives on location at time of inspection. Cellar was filled in and covered with black felt containment. Well had a digital pressure gauge on the tubing that read 486 psi at time of inspection. There was an analog 0-10k psi pressure gauge attached above the swab valve that read approximately 200 psi at time inspection. No violations noted at time of inspection.
2775197 Primary Facility 2018-09-11 Site Restoration No Violations Noted The operator submitted a post-drill well site restoration report for this site which prompted this inspection. A SWN employee arrived at the site simultaneously. Another SWN employee, Tyler Parsons, arrived at the site during the inspection. Mr. Parsons relayed that the five frac tanks on location are in use as temporary additional storage for produced water.

Three of seven permitted wells (Brown 1H, 2H, and 3H) have been drilled and stimulated. These three wells were in production at the time of inspection.

The site is stable with 70 percent vegetative cover overall. A bioretention basin and a rock-lined ditch provide PCSM. These appear to be functioning as designed. The site has been permitted to remain at full pad size.

At this time, due to the presence of three roll off tanks, a battery of five frac tanks used for temporary produced water storage, and secondary containment around the wellheads and frac tanks, the site does not yet meet all of the post-drilling site restoration criteria under 78a.65.

2801742 Primary Facility 2018-11-13 Routine/Complete Inspection No Violations Noted Three wells staged for production. Three each sand and water separators within containment. No leaks.

Pipeline project being run to site at this time.

2842761 Primary Facility 2019-02-21 Routine/Complete Inspection No Violations Noted Weather was 41 degrees and sunny. There were no Southwestern representatives on location at time of inspection. Well had digital pressure monitoring equipment attached to the production tubing. Tubing pressure was 304 PSI at time of inspection. The 9x5 annulus vent was plumbed into the pay line. 0% combustable gas was detected around the well at time of inspection. No violations noted at time of insoection.
