2648864 Primary Facility 2017-10-17 Routine/Complete Inspection No Violations Noted On 10/17/17 at 1310 hrs I conducted an inspection of the Warrant 3914 1906 well site. No one was present at the site. It was sunny and 57 degrees F.

The well was drilled by the Pennsylvania General Energy Co. (PGE) in 2006 and transferred to the Minard Run Oil Company on 3/23/17. The well was tagged. According to DEP records, the well was in production between 2008 - 2014; it has been on inactive status since 12/23/15.

The site is restored and permanently stabilized as per 25 Pa Code Section 102.22(a). A Well Site Restoration Report (WSRR) was received on 11/6/13 and acknowledged on 11/15/13.

No violations were cited during the inspection. I departed the location at 1345 hrs. TLT

2685854 Primary Facility 2018-01-23 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was not in production. The wellhead assembly all fittings and flow lines were tested for gas. The results of this testing was no gas was detected on the wellhead assembly or the well area.
