2496170 Primary Facility 2016-06-24 Routine/Complete Inspection No Violations Noted Routine inspection to assess overall site conditions. No Chief representatives were present.

Vegetative cover on site is greater than 70% total coverage of perennial plant species. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

The well pad surface has been decompacted and revegetated as an interim restoration measure. Title 59 Pa. C.S. § 3216 (2012 Oil and gas Act), which relates to site restoration, requires that a well site be reclaimed within 9 months of completing the drilling of the last well. According to the Department's records, no current well permit exists for this site and no request for a 2-year extension of time for site restoration under the 2012 Oil and Gas Act has been received. In order to remain in compliance with the Department's regulations regarding site restoration, a new well permit should be obtained or a request for an extension of time for site restoration should be submitted.

2559350 Primary Facility 2017-01-31 Routine/Complete Inspection No Violations Noted Arrived @13:25. Weather: cold, snow. No one was on location at the time of my inspection. There was one well on the pad and it was in production. Annuli were plumbed to 550-gallon poly tank which contained ¿ 200 gal of mud/fluid. No combustible gas was detected at the polytank vent. The cellar was overflowing with water and ice.

Top gauge: 1000 psi (analog).

2697585 Primary Facility 2018-02-23 Routine/Complete Inspection No Violations Noted One well staged for production phase. One water separator. No leaks detected from production equipment. No violations noted.
2761041 Primary Facility 2018-08-07 Routine/Complete Inspection No Violations Noted The Department spoke with Rig Managers Steve and Trey. The 513 Rig is drilling the 2H at this time. The Rig could be done drilling in 17 days.
2854503 Primary Facility 2019-03-14 Routine/Complete Inspection No Violations Noted Arrived onsite for a routine inspection at 09:52; the weather was ~45°F and sunny.

There were 3 properly tagged Marcellus wells stage for production. The wells were plumbed to 2 poly vent tanks that were not on secondary containment. The well cellars were filled with water. No bubbling was observed, however, there was a slight sheen on part of the puddle around the wells (Photo #9). There were 3 GPUs and produced water tanks, 1 residual waste container, and 8 baker tanks onsite.

There were rutted areas around the pad and site sign (Photo #1). Overall, the pad was very muddy but no major E&S issues were noted. Areas that were not immediately off the pad surface showed sufficient vegetative cover. There were 3 infiltration basin constructed. The basin contained ice and cattails.

It is recommended that the basins be monitored to determine if they are functioning as designed or if remedial work is needed. It is also recommended that storage units, such as the residual waste container, be placed on secondary
