2051259 Primary Facility 2012-03-13 Drilling/Alteration Pending Falcon 17. Drilling ahead. When drilling out 9 5/8" started seeing bubbling on backside of casing. Shoe test and FIT good. DEP notified on 3/12/12. Operator will be installing packoff assembly at surface and will vent gas. Used Halliburton thixotropic cement blend. Indications are that too much retardant was mixed with blend which caused set time to be delayed. Lab results for compressive strenght have not been received yet. Inspection of cellar revealed steady stream of small bubbles on 13 x 9 annulus. 13 3/8" is cemented to surface. No bubbling observed on 20 x 13 3/8" annulus. Request operator to provide laboratory data on cement when received. Operator shall notify DEP when packoff assembly is installed and to continue monitoring cellar for gas.
2051366 Primary Facility 2012-03-16 Follow-up Inspection Pending Follow up on cement job on 9 5/8". Bubbling is present on 9 5/8" annulus. Have pack off assembly ordered. Co. Man indicated that cement contractor admittedly used too much retardant during job which aggected set up time. Driller waited on cement at least 24 hrs before drilling out. Cement lab results reprtedly indicated 500 psi compressive strength @ 24 hrs. Swell pacer was installed 50' above 13 3/8" shoe, and a swell packer @ 75' below 13 3/8" shoe. 13 3/8" set at 688' and 9 5/8" set at 1669'. DEP will continue to follow up. Rig has moved off location. Crews on site cleaning up containment.

The cellar was full of gravel and there was no gas detected at the perimeter of the cellar. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 528psi on the production casing and 414psi on the production line. There was a 500bbl tank on containment and the containment appeared to be in good condition, there were no lines going to the tank and the tank was empty. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:55p.m.
