2510795 Primary Facility 2016-08-05 Incident- Response to Accident or Event No Violations Noted Afton Sterling, Regulatory Supervisor of SWN, contacted Scott Kuchta, DEP O&G WQS at 9:40 AM on 8/5/16 to report a diesel fuel spill of an estimated (750) gallons to secondary containment that occurred on 8/5/16 at 5:30 AM. Southwestern Energy submitted a drilling resumption notice to the Department on 7/31/16 indicating that Southwestern Drilling Company Rig #45 will be re-entering the Cramer 2H at 18:00 hours on 8/1/16. Mr. Kuchta and I arrived at the site at 12:00 to conduct an investigation of the incident. SDC #45 is currently in the process of drilling the Cramer 2H well. This was a re-entry of the well. The well had been drilled to kick off point. The horizontal section of the well will be drilled to total depth with a mixture of diesel fuel and synthetic oil based mud. Initially, we met with Mike Deming, Senior Regulatory Analyst of SWN at the entrance gate to the site. We briefly discussed the incident prior to visually assessing it. 2513169 Primary Facility 2016-08-22 Routine/Complete Inspection No Violations Noted On 8/22/16, the Department conducted an inspection of the SWN RU 30 Cramer Pad located in New Milford Township, Susquehanna County in response to a second reported release of storm water from the drilling rig well pad containment. The first release occurred on 8/14/16 when heavy rains occurred and resulted in an uncontrolled release of storm water from the containment. David Schoonover, Department OGI inspected the well site on 8/17/16 and found no evidence that anything other than storm water had been released from containment. That inspection was documented under inspection ID # 2511243. Today’s inspection also found that there was no evidence that anything other than storm water had been released. During today’s inspection, SWN was completing the process of raising the containment berms in order to reduce the likelihood of a future storm water release. No violations were noted.
2528642 Primary Facility 2016-10-13 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:40p.m., met with company men Arnando Trujillo and Jonathan Sites. USWS (US Well Services) was on site to frack the Cramer 2H, 3H, 4H, 7H, and the Zeffer 4H wells. Upon arrival, Renegade Services was running a plug and perforation into the Zeffer 4H. The Cramer 2H, 3H, 4H, and 7H were completed and they just finished stage 4 out of 20 on the Zeffer 4H. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 14:45p.m.

2556143 Primary Facility 2017-01-26 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:40p.m., the well was recently completed and is in production. The well tag was on the well and correct. The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) vent pipe was open and set up with a Taylor plug to check for pressure and the valve going to the production tank was closed. There was 716psi on the production casing. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 14:25p.m.

2647434 Primary Facility 2017-10-13 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2669966 Primary Facility 2017-12-11 Routine/Complete Inspection No Violations Noted I performed a routine complete inspection (RTNC) of SWN Well Pad 30 (Cramer) on Monday December 11, 2017. The wells which were on site were as follows:

115-21629 Zeffer N 4h 115-20559 Cramer 6h 115-20675 Cramer 5h 115-21096 Cramer 2h 115-21097 Cramer 3h 115-21099 Cramer 7h 115-21098 Cramer 4h.

The cut and fill slopes were not large and were well vegetated. The pad surface was solid and stable with no evidence of mud being tracked off site. The access road was solid as well. The wells were all clearly labeled and the Cramer 2h was sporting a cage around it. The production water liner was deteriorating but appeared to be watertight. It would be recommended this be checked and repaired asap. There were no violations

2710880 Primary Facility 2018-03-22 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. No E&S issues identified. No violations noted.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
