Well permits that have been issued for this pad are the 1H (115-21223), 2H (115-21224), and the 3H (115-21225). Inspection conducted with Eric Griffin of Don Bower Construction and Dillon Otten field tech for Tioga Environmental Consulting. All perimeter BMP’s in place. Red lined diversion berm reconfiguration located on the northeast side of the pad is acceptable. Pad is approximately 80% complete.

Inspection conducted in response to a complaint that pad design and installation is causing increased storm water flow to agricultural field. Ground is frozen and snow covered. Pad placement does not appear as if would increase storm water flow to field in question.
2238465 Primary Facility 2014-01-23 Administrative/File Review Pending On 1/21/14, Andy Mehalko reported the following incident: On 1/20/14 at approximately 10:00 AM a release of approximately 5-6 gallons of glycol solution was identified at the Pritchard 1H well. Due to an issue with the thermostat on the GPU, the burner unit overheated the glycol solution causing it to flow, as designed, into the adjacent overflow tank. At this point, due to the volume and temperature of the fluid transferred, it escaped the primary overflow tank and was captured by the secondary containment system consisting of an overpack type drum. Unfortunately, the drain valve at the bottom of the overpack drum was not fully seated allowing a portion of the fluid to migrate to the pad surface.

Pad documented in the 1/23/14 inspection record # 2238465. A review of the remediation summary report prepared by Resource Environmental indicates that the remediation was adequate. No further action is required of Cabot at this time. Attached is a copy of the remediation summary report, the waste manifest associated with this spill, and the 1/23/14 inspection.

2533885 Primary Facility 2016-11-01 Routine/Complete Inspection No Violations Noted On 11/1/16, the Department conducted an inspection of the COG Pritchard D Well Site located in Harford Township, Susquehanna County in response to a nearby malodor complaint. The Department arrived on-site at approximately 12:00 AM. There are 2 wells drilled and in production on this pad, the 1H and 2H. The pad is equipped with the standard equipment associated with steady state production. A water truck was collecting produced fluid from the production tanks during the inspection. No other activity is occurring at this pad. I spent approximately 5 minutes at each corner of the well pad and did not detect any malodors. No E&S issues were observed during this inspection. No violations noted.

The cellar was about a quarter full of water with no bubbling noticed. The (13x9) annulus vent pipe was open and venting into the production tank, the (9x5) annulus vent pipe was open and venting into the production tank. There was 498psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:45p.m.

There were no violations noted.

Production tank containment should be emptied the next time the stormwater melts, as there is not much free board present. Overall the site has good stabilization. No E&S issues identified.
