The presumed 1H was not visibly tagged at the time of inspection. Bottom portion of stack was set on casing. All casing valves were closed with 0 psi on gauges. Cellar was covered in snow. Drillers log in mailbox indicated this well had 740' of 133/8 casing cemented to surface and 1618' of 95/8 casing cemented to surface. BOP was tested to 3000psi. The 2H had bop set on 95/8 casing. Drillers log indicated this well has 710' of 133/8 casing cemented to surface and 1614' of 95/8 casing cemented to surface. Yellow tag on head indicated hole was filled with 433 barrels of obm at 12ppg. Stack tested to 3000psi. The 3H had bop in place, yellow tag on stack indicating this well has 718' of surface and 1656' of 95/8 casing. Both tophole strings are cemented to surface. Hole was loaded with 433 barrels of 12 ppg OBM. BOP tested at 3000psi.

2521329 Primary Facility 2016-09-20 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 12:55p.m., I met with company man Rick Sizemore, SDC Rig 45 was on site, they finished drilling the hole for the production casing and was pulling the drill pipe out of the hole. The top hole section of the well was drilled and completed on 12/12/14. We took a walk around the location and everything looked OK.

The depth at time of inspection was 11113'. The projected TD is 11113'. I left the site at 13:50p.m.

2557396 Primary Facility 2017-01-31 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 11:40a.m., US Well Services was on site to frack the 1H, 2H, 3H, and 7H wells. I met with company men Donnie Torres and Troy Wilson. They were fracking stage 6 out of 21 stages on the 3H, the 1H and 7H were both completed with 21 stages in each well, the 2H hasn’t been started yet with 17 stages. The water is being pumped in from the Zeffer impoundment. I took a walk around the location and there were no obvious concerns noticed.

I left the site at 12:25p.m.

2596350 Primary Facility 2017-05-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 8:55a.m.

The well was recently completed and is in production. The well tag was on the well and correct. The cellar was set up with a plastic corrugated pipe around the well for inspection access; there was water above the surface casing with no bubbling noticed. The (9x5) annulus vent pipe was open and set up with a Taylor plug to check for pressure with a Crystal pressure gauge, the valve going to the production tank was closed. There was 691psi on the production casing. I took a walk around the location and there were no obvious concerns noticed. I left the site at 9:45a.m.

2648685 Primary Facility 2017-10-04 Incident- Response to Accident or Event No Violations Noted The Department (Briana Cunningham and I) arrived at the site at 10:48. The weather conditions are sunny skies with temperatures in the low to mid 60's, with a slight breeze. There was no activity at the site. We met with Seth Tratthen, Regulatory Analyst of SWN. Mr. Tratthen stated the following in reference to the spill. "The clamshell on the centrifugal pump had a 3" crack in it. The pump leaked fluid into the containment and eventually overflowed onto the pad surface. The pump has been removed from the pad. The last containment inspection was on 9/27/17 and no issues were noted. The fluid spilled originated from the nearby SWN Zeffer large Impoundment. The fluid was a mixture of freshwater and production fluid pumped through an underground pipeline from the Zeffer to this pad to be boosted with the pump. A vacuum truck was on site on 10/3/17 and it recovered 50bbl (barrels) from the containment, ground, primary containment from the pump on the trailer, plumbing, and the risers. 2697758 Primary Facility 2018-02-14 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was in production. The cellar was set up with a corrugated pipe around the well for inspection access. The (20x13) and (13x9) annuli were below the water level and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2774348 Primary Facility 2018-09-12 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Standing water on the pad surface was tested for conductivity with all results confirmed to be less than 160 uS. Good, permanent stability of the site exists. No violations noted.
2793128 Primary Facility 2018-10-23 Routine/Complete Inspection No Violations Noted At time of inspection.

Wyatt Tompkins (Regulatory Technician) for SWN was on site and using a flare camera to detect gas leaks at the well heads and GPU units that would otherwise be undetectable. The well was in production. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water .5' below grade in the cellar and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
