I met with company man John Hill. Schlumberger was on site to cement the intermediate casing string. Schlumberger started their procedure for cementing the casing, they were shut down because of lightning before cement was started. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:45p.m.

2624149 Primary Facility 2017-08-08 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 9:45a.m.

I met with company man Joe Baker. SDC Rig #41 was on site, they finished drilling the lateral section of the well and was running production casing. The top-hole section of the well was drilled and completed by WSG Rig #1 on 6/7/17 to a depth of 1635'. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 11073'. I left the site at 10:50a.m.

2639411 Primary Facility 2017-09-25 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company man Harry Stevens. Halliburton was on site to frac the 1H, 2H, 4H and 5H wells. They were on the 4H, they had 11 out of 32 stages completed and was waiting on sand before starting stage 12. The 1H and 2H wells haven't been started. They will both have 18 stages. The 5H is completed with 32 stages. The water is being pumped to the site from the Zeffer production water impoundment. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2680871 Primary Facility 2018-01-10 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The well tag is on the well and correct. The cellar is set up with a corrugated pipe around the well for inspection access. There was ice above the (20x13) and (13x9) annuli and was unable to check for gas. There were 5 empty 500bbl water tanks on containment and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2687986 Primary Facility 2018-01-23 Incident- Response to Accident or Event No Violations Noted Today's inspection was conducted as an incident inspection. At approximately noon today, Mike Deming, SWN Senior Regulatory Analyst contacted the Department to inform the Department that SWN had identified stormwater discharges off of secondary containment on 3 of their pads, of which this pad is one. The Department met Mr. Deming and Dillon Otten, SWN Regulatory Analyst on-site at approximately 1:00 PM. Mr. Deming stated that the containment shown in photo's 2 and 3 is containment that is no longer in use, and that it is more than likely going to be eliminated. The water tank within the containment was empty. SWN has eliminated the discharge via vac truck. The Department conducted conductivity meter readings and found conductivity at 54 uS outside containment and 10 uS within containment. The readings are indicative of stormwater and finds no violations associated with discharges currently associated with said discharge. No E&S violations were noted during this inspection. No violations noted.
