2534063 Primary Facility 2016-11-01 Preoperation Inspection No Violations Noted On 11/1/16, the Department was on site at the SWN RU 74 Tretter Well Pad to conduct a Preoperational Inspection. Inspection conducted with Rich Gulyas, SWN Construction Foreman, Andy Breese, SWN Construction Foreman, and Seth Tratthen, Regulatory Analyst. The well permits that have been issued for this pad are the Tretter 1H (115-22140), the 2H (115-22141), and the 3H (115-22142). All perimeter BMP’s in place to include red lined additions. Pad is 100% complete. Approximately 75% of slopes rock stabilized. No violations noted.
2575793 Primary Facility 2017-03-21 Routine/Complete Inspection No Violations Noted The Department was on site to conduct an inspection. At the time of the inspection, the site was covered in snow. It was noted that clearing has been started for the pipeline adjacent to this site. Additionally, on 3/20/17 the Department received notification that the SPUD notification for the 2H and 1H wells have been deleted.
2700735 Primary Facility 2018-03-05 Drilling/Alteration No Violations Noted At time of inspection.

I was on site for a new spud inspection. Minichi Contracting Group was on site, they drilled and set the conductor pipe for the Tretter 1H and 2H wells with a dual rotary rig. I met with driller Keith Bowman. The 1H was drilled and set at 88'. The 2H was drilled and set at 75'. Drill cuttings were put in a sealed rolloff container and hauled to the Keystone landfill. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2712043 Primary Facility 2018-03-29 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Johnny Hale. Falcon 24 was on site and drilling the vertical section of the well to kick off. Proposed TD for the vertical section is 5600'. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2740022 Primary Facility 2018-06-11 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company man Jeff Voltz. USWS was on site to frac the Tretter 1H, 2H and Walker West 1H. The Walker West 1H was completed with 24 stages. The Tretter 1H was completed with 40 stages. There were 35 stages completed out of 45 on the Tretter 2H. They were shut down and waiting for a coaled tubing rig when I arrived on location. The plug was set high after stage 35 at 8013' with perforations above the plug in the Tretter 2H, they plan on drilling out the plug and resume stimulation on the well. The water is being pumped to the location from the Young fresh water impoundment and some being pumped from the flow back at the Breese pad. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2744322 Primary Facility 2018-06-14 Routine/Complete Inspection No Violations Noted The Department arrived at 1415 to perform a RTNC inspection on the pad. Present to represent the Department were Gene Rickard (WQS-DEP) and myself.

Upon arrival at the site, Mr. Rickard and I signed in at the guard station. We asked the guard what type of activity was currently being performed at the pad, as we did not want to enter the site if well stimulation was actively being performed. She informed us that well stimulation had been underway earlier in the week, but the crew had encountered a problem and had put well stimulation on hold while they corrected it. She was not aware of the exact nature of the problem.

After parking our vehicle, we tracked down Travis Dye (Well Site Supervisor-SWN) in order to find out exactly what type of problem they had encountered and what type of corrective actions were being performed. Mr. Dye informed us that while performing well stimulation activities on the 2H, SWN discovered one of the plugs had not been set properly. In order to continue well stimulation, the

2745271 Primary Facility 2018-06-20 Routine/Complete Inspection No Violations Noted Today's inspection was conducted in response to SWN's self reporting of the failure of secondary containment emplaced for the containment of the tank battery that had been set up for fracking operations. The containment failed during a storm event and released what was reported to be approximately 4200 gallons of rainwater. As photo's 3 and 4 show, some gravel material had been mobilized, but none was observed to have left the site. After flowback operations are complete, the Department expects SWN will repair the minor damage that has occurred in this location. Well tender Shane Beamer stated that fracing operations finished up either yesterday or the day before. The original 3 wells have just been placed back into production. Production equipmment for the other 3 new wells is almost complete. Flow back operations on the wells have not commenced yet. No violations were observed during this inspection.
2771103 Primary Facility 2018-09-04 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The well tag was on the well and correct. The cellar was set up with a plastic corrugated pipe around the well for inspection access. There was water 3' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. There were 18 empty 500bbl tanks on containment and the containment appeared to be in good condition. There was a single empty 500bbl tank not on containment. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2819966 Primary Facility 2018-12-27 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. All 6 wells appear to be in production. No E&S issues identified. Good vegetative stabilization present. Site is approximately 80% stone stabilized. No violations present.
