Minichi Contracting Group drilled the holes and ran the conductor pipe with a dual rotary rig on the 1H and 2H wells. I spoke with welder Mylo Welter of Minichi who was welding caps on the conductor pipe, and a backhoe was digging out around the conductors to set the cellars. I checked the drillers log book to get the depth of the conductor pipe, the 1H and 2H were both drilled and set to 70'. The drill cuttings were put into a sealed role off container and hauled to the Keystone Landfill. I took a walk around the location and there were no obvious concerns noticed. I left the site at 9:25a.m.

2607529 Primary Facility 2017-06-22 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 14:20p.m.

I met with company man Robert Gray. W S G Rig #1 was on site and Schlumberger was cementing the intermediate casing. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 1638'. I left the site at 15:45p.m.

2618608 Primary Facility 2017-07-24 Drilling/Alteration No Violations Noted At time of inspection, arrived on site at 13:45p.m.

I met with company man Andrew Griffith. SDC Rig45 was on site and drilling the lateral section of the well for the production casing. The top-hole section of the well was drilled and completed on 6/23/17 by WSG Rig #1 to the depth of the intermediate casing. I took a walk around the location and there were no obvious concerns noticed. Depth at time of inspection was 12,439'. Projected depth for the well is 12,555'. I left the site at 14:25p.m.

2629657 Primary Facility 2017-08-22 Routine/Complete Inspection No Violations Noted At time of inspection, arrived on site at 13:30p.m.

I met with company man Harry Stevens. Halliburton was on site to stimulate the 1H and 2H wells. They completed 2 stages and was starting stage 3 out 34 stages on the 1H. They completed 6 stages out of 28 stages on the 2H. Stage 6 pressured up at the end of the stage, so they were unable to pump the plug and perf down for stage 7. They will finish the 1H then go back to the 2H. The water was being pumped to the site from the Zeffer production water impoundment and Odell fresh water impoundment. I took a walk around the location and there were no obvious concerns noticed. I left the site at 14:20p.m.

2661379 Primary Facility 2017-11-16 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put into production. The well tag was in place and correct. The cellar was set up with a corrugated pipe around the well for inspection access and there was no bubbling noticed. I took a walk around the location and there were no obvious concerns noticed.

There was water 1.5' below grade in the cellar with no bubbling noticed and there was no gas detected in the remainder of the cellar. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
