The last inspection conducted on behalf of the operator was on 5/7/12 by Penn E&R. There was 17 wheelie tanks on-site and they were on containment. Several puddles on the pad (not on containment) were screened for elevated conductivity, none exhibited high readings. One area on containment had a reading of 4.27 mS. This was located near the wheelie tank N37013. There were multiple holes observed in the liner, but none in the area of the elevated reading. The cause of the elevated reading should be determined and corrected. The liquid on the liner should be collected and disposed of properly. The integrity of the liner should be retired as soon as possible. No erosion issued were observed during this inspection. No new violations were cited during this inspection. - MRN
2201786 Primary Facility 2013-09-03 Routine/Partial Inspection No Violations Noted An inspection of the Showalter 822 site in Chatham Township, Tioga County was conducted on September 3, 2013. The last inspection conducted on behalf of the operator was on 8/19/2013. The 1H, 2H and 4H wells were drilled and appeared to be in production, one additional well was spudded. No puddles exhibiting elevated conductivity or erosion issues were observed at this time. All area were well vegetated or otherwise stabilized, no issues were noted. 2442843 Primary Facility 2016-01-20 Complaint Inspection No Violations Noted The Department inspected the SWEPI LP site – Showalter 822 (ESX11-117-0011) located in Chatham Twp., Tioga Co. on 1/20/2016 @ 1115 hours. The weather was 18° and overcast with blustery winds and a dusting of snow and ice. A copy of the E&S plan was located in the mailbox along with the onsite inspection log, well permits and PPC Plan. The site was last inspected by company personnel on 1/14/2016 with “site snow covered” listed in the comments section. Total Project area is listed as 20.20 acres. The receiving stream is listed as “UNT to Crooked Creek/Susquehanna River Basin in PA” WWF designation.

2729673 Primary Facility 2018-05-14 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the SWEPI LP-Showalter 822 Well Pad located in Chatham Township, Tioga County on May 14, 2018 at 1300 hrs. the weather was 73° and sunny. Winds WSW @9 mph, 53% humidity.

The current ESCGP-2 authorizes the expansion of the existing well pad by adding a 41,241 ft² section to the west, northwest, and south west side of the pad as well as the addition of a 200'x 200' tank pad. The well pad was previously in interim restoration with 4 wells observed drilled, 3 of which appear to be in production.

As of today's inspection contractor JL watts is in the process of expanding the pad. Temporary erosion and sediment control's appear to be installed as specified in the plan drawings. No erosion or sedimentation was observed.

2775759 Primary Facility 2018-09-17 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the SWEPI LP - Showalter 822 well pad located in Chatham Twp, Tioga Co. on 917/2018 @1245 hours. The weather was 72 degrees and cloudy, winds N @11mph, 94% humidity. Present for the inspection was myself and Korey Starkey, DEP Env Trainee.

The access road, well pad, and E&S controls were observed. The current ESCGP-2 authorized the expansion of the existing well pad by adding a 41,241 ft² section to the west, northwest, and south west side of the pad as well as the addition of a 200'x 200' tank pad.

The well pad was previously in interim restoration with 4 wells observed drilled, 3 of which appear to be in production. 4 new well cellars were observed on the pad surface. An infiltration basin was observed below the north side of the well pad. Temporary BMPs were observed in the form of CFS below the newly constructed pony pad and below the pad expansion area. No erosion or sedimentation was observed onsite.
