2361300 Primary Facility 2015-04-15 Routine/Complete Inspection No Violations Noted Temporary stabilization generally >70% cover. Temp BMPs in place. One filter sock near access road entrance needs repair. Elevated conductivity in sediment/infiltration basin on north side of pad (2.3 mS/cm). Several stained spots on pad surface.
2717066 Primary Facility 2018-04-11 Routine/Complete Inspection No Violations Noted Weather was 34° and sunny at time of inspection. Well is shut in and not tied into any service plumbing. There are no pressure gauges attached to the well. Well has three annulus vents plumbed to surface that are 2", 1 1/2" and 1" in diameter. The 2 inch annulus vent had 25% methane detected at time of inspection. The other two vents had 0% combustible gas detected. No violations were noted at time of inspection.
2738689 Primary Facility 2018-06-05 Routine/Complete Inspection Pending I met Luke Chesner and Levi Lee of Rockdale when I arrived. Coil tubing rig on location drilling out plugs on the 6H prior to moving to the 4H. I noted two areas on the pad that were wet and muddy, one between the coil tubing rig and a pump truck (approximately 5-8 gallons of free water and 30' x 40'), and one between two Baker tank batteries (approximately 10-15 gallons of free water and 30' x 50'). Conductance reading in the former was 9.22 mS/cm, and was over the limit of my meter (19.99mS/cm) in the latter. I pointed both of these areas out to Luke and Levi, and took a grab sample (ID#0939066) from a puddle in the area between the Baker tank batteries. Luke and Levi had a vac truck working in the area between the pump truck and the coil tubing rig almost immediately, and I recommended that they vac the area between the Baker tank batteries and start to remove the affected soil ASAP. The elevated conductance in these areas is an indication of a potential release of produced water or flowback to the
2745372 Primary Facility 2018-06-26 Administrative/File Review Violation(s) Noted This is an administrative inspection generated due to receiving final sample results from the Bureau of Laboratories. Elevated barium, strontium, and chlorides in the sample result indicate a release of wastewater to the pad surface. Unless Rockdale can provide an explanation of why they believe that the spill was less than 42 gallons, the site must be remediated under Act 2 as required by 25 PA Code 78a.66(c)(2).
2760767 Primary Facility 2018-08-06 Incident- Response to Accident or Event Violation(s) & Outstanding Violations I arrived on site in response to a spill notification from Rockdale called in at 09:45 on 8/4/18. Regina Logue of Rockdale reported that approximately 2 barrels of potentially contaminated rainwater was released to the pad due to a failed containment, and stated that the release was near the aboveground storage tank. The only person present at the pad when I arrived was Brady Bryington of Principal Enterprise. He pointed out the area of the release, and when I inquired informed me that the AST contained fresh water. I screened the release area, which was at the corner of an upright tank containment, with my soil conductance meter; most readings were in the range of .10-2.2 mS/cm, and the readings dropped dramatically when I pushed the meter 1/4" into the soil. I did locate a spot that seemed to be somewhat removed from the spill area near the center of the upright tank containment that was over limit on my soil meter (greater than 4.0 mS/cm). Towards the rock-lined ditch from the release area, soil con
2790953 Primary Facility 2018-10-17 Routine/Partial Inspection Outstanding Violations - Viols Req'd No activity on site. Due to time constraints, only a partial inspection was performed. Site had a large puddle (50-100 gallons) present in the area of the release documented in the 6/5/18 inspection. Soil conductance readings in this area ranged from .20-.53 mS/cm. Violations remain outstanding.
