2854060 Primary Facility 2019-03-14 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Flack/McConnell 502 Well Site in Sullivan Twp, Tioga County on 3/14/2019 at 1230 hours. The weather was sunny and 56 degrees. The act 9 sign was observed at the entrance to the site. The E&S Plan and inspection log were available for review in the mailbox at the entrance. Company personnel last inspected the site on 3/12/2019 noting no issues.

There are currently 3 wells onsite in producing, the 2H, 4H & 6H. The site has very steep slopes but they are vegetated very well and no erosion was observed. PCSM BMPs are installed per plan and functioning. Containment structures are intact and functioning and no leaks or releases were observed. Puddles on the pad were within normal conductivity ranges.

No violations or issues noted today, I departed the site at 1310.
