2856976 Primary Facility 2019-03-21 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Sherman 492 Well Pad in Sullivan Twp, Tioga County on 3/21/2019 at 1140 hours. The weather was cloudy and 44 degrees. The act 9 sign was observed at the entrance to the site. The E&S Plan was available for review in the mailbox at the entrance. The inspection log was in the mailbox by the production tanks. Company personnel last inspected the site on 3/18/2019 noting no tank or containment issues.

There are currently 4 producing wells onsite(1H-4H). The site is well vegetated and stabilized except for the access road. See findings section "Site Stabilization." Containment structures were intact and no leaks were detected. No evidence of spills or releases to the pad surface were observed. Puddles on the surface were found to be within normal conductivity ranges. All planned PCSM BMPs are installed and functioning properly.

No violations noted today. I departed the site at 1230.
