2367321 | Primary Facility | 2015-05-06 | Administrative/File Review | No Violations Noted | The Department has reviewed the Completion Report for the removal of the high conductivity pad material at the above-referenced well pad that occurred on January 7, 2015 and the release of approximately 10 gallons of flowback to the pad surface on January 29, 2015 at the same location. The report was prepared by SWEPI LP and received by the Department on April 16, 2015. The review was conducted in accordance with the Alternative Remediation provision of the Department policy document 800-5000-001 Addressing Spills and Releases at Oil and Gas Well Sites or Access Roads. No technical deficiencies were noted. Response actions conducted under the Alternative Remediation provision do not qualify for the relief of environmental liability provided by the Land Recycling and Environmental Remediation Standards Act. |

2644722 | Primary Facility | 2017-09-29 | Site Restoration | No Violations Noted | A puddle on the pad gave electrical conductivity of 1.0 mS/cm wit an Oakton PCSTestr 35. I estimate the flat pad area at 2.7 acres. JLP |
2812620 | Primary Facility | 2018-12-10 | Routine/Complete Inspection | No Violations Noted | The DEP conducted a routine inspection of SWEPI's Shughart 490 Well Pad in Richmond Twp, Tioga County on 12/10/2018 at 1205 hours. The weather was cloudy and 30 degrees.
There are three producing wells onsite with one highland production tank. The site appears to still be stabilized and no erosional issues were observed. Fluids on the pad surface were frozen. The long, steep access road to this pad and the Shughart 534 have numerous waterbars in place. Only minor erosional channels were observed on the access road, presumably due to the heavy amounts of rain throughout the year. No violations noted today, I departed the site at 1230 hours. |
