2490523 Primary Facility 2016-05-25 Site Restoration No Violations Noted A site restoration inspection resulting from an operator submittal of a Notice of Termination (NOT), in accordance with 25 Pa. Code § 102.7, for the ESCGP-1 (ESX09-125-0068) that regulated earth disturbance activities associated with the Franklin Lakeview Estates Well Sites.

Inspection of each well site confirmed that all requirements established in 25 Pa. Code §102.22(a), regarding permanent stabilization, have been attained. I recommend the approval of the NOT request for ESCGP-1, ESX09-125-0068.

2648936 Primary Facility 2017-10-18 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Franklin Lakeview Estates well pad and the following was observed.

Proper signs were displayed identifying the site. The site was stabilized with thick vegetation no erosion observed outside of the LOD. The well heads were maintained, no leaks or fluids around well head. The production tanks were in good condition no leaks , rust or defects observed. Secondary containment for the site was intact and maintained.
