2810008 Primary Facility 2018-10-23 Site Restoration No Violations Noted Today I conducted a routine inspection at the Burnswoth well pad and the following was observed.

Proper signs were displayed identifying the site. The access road was maintained and in good condition. The outer slopes were stabilized with thick vegetation no erosion observed outside of the LOD. The well heads were maintained no leaks or defects observed. The production tanks were in good condition no defects or leaks.The production tanks secondary containment was intact.

The GPU for the 11H did not have secondary containment. The well was drilled and producing in 2012 prior to the 2016 74a.64a(a) regulation.
