2317328 Primary Facility 2014-07-09 Preoperation Inspection No Violations Noted The pre-operation inspection for the Mad Dog 20/20 well site was conducted with Erin Spine on July 9th, 2014 pursuant to 58 Pa C.S. Section 3258 (a.1). The Mad Dog 20/20 well site has been permitted under ESCGP-2 (ESX14-125-0010) and is located in West Pike Run Township, Washington County.
2327920 Primary Facility 2014-12-10 Complaint Inspection No Violations Noted Rig on site.
2347588 Primary Facility 2015-02-27 Routine/Complete Inspection No Violations Noted All proper records on-site including well permits, ESCGP-2, E&S plan, PCSM, ERP, PPC and containment plan. Act 9 signage in place. The rig is being moved off-site since 2/23/15; the last well (0H) reached lateral TD on 2/21/15. A panel of the sound wall has been removed to tie-in the gathering pipeline spur and will be replaced within the next few days once line is complete. Fraccing is estimated to take place around mid-March. The site is currently snow covered; however it appears that all E&S controls remain in place and are being maintained. The site appears to be stable and no violations were noted during this inspection.
2373237 Primary Facility 2015-04-23 Routine/Partial Inspection No Violations Noted On 4/23/15 water samples were taken from well for 946 analyses.
2373260 Primary Facility 2015-05-26 Administrative/File Review No Violations Noted On 5/26/15 administrative review of lab results with WQS Supervisor John Carson indicates that gas well activities have not impacted the water supply at this time.
2462470 Primary Facility 2016-01-06 Routine/Complete Inspection No Violations Noted
2505811 Primary Facility 2016-05-25 Complaint Inspection No Violations Noted On 5/25/16, water samples were collected for SAC 946, bacterial count, and UVIR analysis in response to a complaint (319774) regarding the Mad Dog 2020 site.
2506892 Primary Facility 2016-06-16 Administrative/File Review No Violations Noted Administrative review of lab results on 6/16/16 indicates that water supply does not appear to have been impacted by oil & gas activity. Results and documentation for complaint 319774 forwarded for final determination.
2516098 Primary Facility 2016-04-13 Routine/Partial Inspection No Violations Noted On 4/13/16, I performed a routine partial inspection at the Mad Dog 2020 site accompanied by WQS Supervisor John Carson in response to complaint 319062 and made the following observations:

•Site restoration work including the construction of infiltration berms was being conducted at the time of inspection. •Additional servicing and work was being conducted on the adjacent pipeline connection at the time of inspection. •Vegetation on the cut slope above the pad was sparse and patchy. No violation.

2516656 Primary Facility 2016-04-26 Follow-up Inspection No Violations Noted On 4/26/16, I performed a follow up inspection at the Mad Dog 20/20 site in response to observed conditions noted in a prior inspection on 4/13/16 and made the following observations:

• Temporary stabilization was found in place on all disturbed areas on site. • PCSM control implementation was completed at the time of inspection. • Disturbed areas along the adjacent pipeline were not stabilized at the time of inspection. No violation.

2698769 Primary Facility 2018-02-27 Incident- Response to Accident or Event No Violations Noted On 2/27/18 an incident inspection was conducted at the Mad Dog 2020 Well Site in response to an operator reported slide on the cut slope above the pad. The following was observed:

>>The operator reported substantial ground movement a cut slope of the pad in a location that had multiple previous slides. Site inspection found a large area had slid a small amount with no BMP failure and no impact outside of the site LOD. No violation.

>>The operator requested approval for a minor modification to repair the slide including removing slide material to an on site topsoil pile and replace with coarse stone. All exposed soil to be stabilized and protected with CFS. The modification was approved.

2705447 Primary Facility 2018-03-14 Routine/Complete Inspection No Violations Noted Inspected well prior to reviewing Application for Inactive Status. This well has 26" conductor set at 40 ft. No other casing has been set. Well and location appear to be well maintained.
2837945 Primary Facility 2019-02-05 Routine/Complete Inspection No Violations Noted On 2/5/19 a complaint inspection related to the Mad Dog 2020 Well Site was conducted in response to a reported water supply impact. The following was observed:

>>The pad was minimally active at the time of inspection and the site was in generally good condition with no noted spill or erosion issues.

>>The complainant reported recent third party testing indicated multiple elevated parameters. Sample 3401-080 was collected from an outdoor tap per standard procedure.
