2528999 Primary Facility 2016-09-07 Complaint Inspection No Violations Noted On 9/7/2016, I performed a complaint inspection at the Rumpelstiltskin site in response to complaint ID 322302 accompanied by Rice Representative Jonathon Davis and made the following observations:

•Per the Mr. Davis the PCSM construction on the site was temporarily obstructed by larger than estimated amounts of earth needing excavated and hitting bedrock in unexpected areas. Per Mr. Davis a major modification to the permit was being pursued to remediate the situation. •Multiple large areas of the site were unstabilized at the time of inspection; perimeter controls were all in place and appeared functional at the time of inspection. •Necessary site plans and permits were present in the mail box at time of inspection.

2530631 Primary Facility 2016-09-08 Complaint Inspection No Violations Noted On 9/8/16, I performed a complaint inspection associated with the Rumpelstiltskin site in response to complaint ID 322302 and made the following observations:

•I met with Moody’s Water Service at the complainant’s home to collect water samples for 946 analyses. As the complainant was already on supplied water with the well disconnected Moody’s equipment was used to collect and split samples. •Water samples were taken directly from the well after flushing for 10 minutes. •The complainant’s well is located within 1000 feet of the nearest well on site; the wells on the site have initial production reported for February or March of 2016.

2530979 Primary Facility 2016-10-18 Administrative/File Review No Violations Noted On 10/18/16, I performed an administrative inspection associated with the Rumpelstiltskin site in response to complaint ID 322302 and made the following observations:

•WQS Supervisor John Carson and I reviewed the laboratory results for the samples taken 9/8/16 as well as other situational information and found that, due to changes in water quality from predrill samples and the status of the site being within presumption both in time and distance, an impact to the complainant’s water supply is presumed. •It is noted that the operator had already provided an alternative water supply at the time of the 9/8/16 inspection at the request of the complainant.

2746799 Primary Facility 2018-06-28 Routine/Complete Inspection No Violations Noted This well has been completed and is producing. Wells and site appear to be well maintained.
2759639 Primary Facility 2018-07-31 Routine/Partial Inspection No Violations Noted On 7/31/18 a routine partial inspection of the Rumpelstiltskin site was conducted in response to an operator request. The following was observed:

>>EQT Representative Jason Tyler requested a meeting to review plans to remove multiple sections of site CFS at landowner request to allow livestock access. Site vegetation suggests that CFS should be removed independent of landowner request.

>>Site appeared in generally good condition with no noted issues.

2761059 Primary Facility 2018-03-30 Incident- Response to Accident or Event In Compliance with Policy On 3/30/18 an incident inspection was conducted of the Rumpelstiltskin Well Pad in response to a spill reported on 3/28/18 (CTS ID: 333271). The following was observed:

>>The operator reported a spill of less than 20 gallons of glycol.

>>Site inspection found that cleanup had been completed with no indication of residual impact or contamination.

>>Four wells were noted in operation at the time of inspection.

>>Site BMPs showed signs of recent maintenance and fresh stabilization. BMPs were functional but close to capacity.

2856904 Primary Facility 2019-03-21 Routine/Complete Inspection No Violations Noted Act 9 site identification sign installed. Well drilled and equipped for production. Well tag present. Well apparatus,brine tanks and gas production unit appears to be in sound condition.
