Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2453130 Primary Facility 2016-02-22 Preoperation Inspection No Violations Noted Pre-operational inspection conducted at the request of the operator in accordance with 58 Pa. C.S. §3258 (a.1). No violations noted during the inspection.
2467089 Primary Facility 2016-03-22 Drilling/Alteration No Violations Noted Cellar Tech auger rig

Drilled a 30” wellbore and set 26” conductor casing at 40 ft. on 3/17/16.

2496396 Primary Facility 2016-06-23 Drilling/Alteration No Violations Noted Alpha Hunter Drilling Rig #6

Moved onto this well on 6/18/16 and resumed drilling at 50 ft. KB. The 18 5/8” casing was set at 678 ft. on 6/21/16 and cemented to surface with 40 bbls returned. The 13 3/8” casing was set at 935 ft. on 6/21/16. The operator attempted to cement casing to surface but got no returns. Tagged cement at 150 ft. and grouted to surface. Coal was found from 885 ft. to 891 ft. A basket was set at 852 ft. The 9 5/8” casing was set at 2575 ft. on 6/23/16 and cemented to surface with 42 bbls returned. A Casing Annulus Packer was set in the 9 5/8” casing string at 1791 ft. Currently at 2625 ft. and waiting on cement.

2500401 Primary Facility 2016-07-07 Drilling/Alteration No Violations Noted Alpha Hunter Drilling Rig #6

KOP was at 6450 ft. TD was at 7402 ft. on 6/26/16.

2704995 Primary Facility 2018-03-07 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Cogar well pad and the following was observed.

Proper signs were displayed identifying the site. EQT currently has a rig on site and is drilling 1 of 6 new wells. The well heads onsite are contained inside subsurface vaults covered with equipment for the drilling process and could not be visually inspected. The production tanks were in good condition no leaks , rust or defects observed. Secondary containment for the site was intact and maintained.

The site had some stabilization issues on the SW side of the access road. I there was some slides forming inside and outside the LOD. I spoke with Brian Morris from EQT about the issue and we arranged a site inspection on 3/13/18 at 1:30pm to discuss the plan for stabilizing that area.

2710040 Primary Facility 2018-03-26 Drilling/Alteration No Violations Noted Pioneer Drilling Rig #72 moved onto this well on 3/26/18. Currently rigging up prior to TIH to resume drilling.
2714238 Primary Facility 2018-04-04 Drilling/Alteration No Violations Noted Pioneer Drilling Rig #72 resumed drilling at 7402 ft. KB. KOP was at 6450 ft. (with air rig). EOB was at 8445 ft. MD (7874 ft. TVD). TD was at 20,765 ft. MD (7685 ft. TVD) on 4/2/18. Currently TIH with tri-cone bit to continue cleaning well bore prior to running 5 1/2" casing.
2717021 Primary Facility 2018-04-11 Drilling/Alteration No Violations Noted Pioneer Drilling Rig #72. Operator unable to get 5 1/2" casing beyond curve. After not being able to clean well bore operator decided to plug pack and sidetrack this well. Currently TOOH with metal tubing after setting 2 isolation plugs. Will TIH with fiberglass tubing to set kick plug.
2719763 Primary Facility 2018-04-18 Drilling/Alteration No Violations Noted Pioneer Drilling Rig #72. Set kick plug above 2 isolation plugs on 4/11/18. TIH and tagged cement at approximately 6150 ft. Found that cement was not hard enough to kick off for sidetrack. Got to 6665 ft. and decided to TOOH and reevaluate plug and sidetrack.
2724074 Primary Facility 2018-04-26 Drilling/Alteration No Violations Noted Pioneer Drilling Rig #72 reentered this well on 4/24/18. Did clean out run to 6500 ft. TOOH to pick up fiberglas tubing. TIH to 6472 ft. and pumped cement. Estimated top of kickoff plug at 4718 ft.

Note: At approximately 4:22 am on 4/26/18 operator had a spill of approximately 1.5 bbls of oil-based drilling fluid after cementing with 1 bbl on containment and 1/2 bbl off containment. Spill was possibly due to a valve that opened on a cement vac truck because of a mechanical failure. Spill was cleaned up prior to inspection.

2727379 Primary Facility 2018-05-08 Drilling/Alteration No Violations Noted Pioneer Drilling Rig #72. Did not hit good cement at plug. Set additional cement plug on 4/28/18. TIH and tagged cement at 4670 ft. MD. Started sidetrack and drilled to 5860 ft. MD. Had issues with MWD surveys. Ran gyro. Stopped drilling, TOOH and moved to next well. Operator will evaluate and resume at later date.
2731452 Primary Facility 2018-05-18 Drilling/Alteration No Violations Noted Pioneer Drilling Rig #72 reentered this well (for the second time) on 5/17/18. Another kick plug was set from 5850 ft. to 5350 ft. on 5/17/18. Currently TIH to start 2nd sidetrack.
2734457 Primary Facility 2018-05-25 Drilling/Alteration No Violations Noted Pioneer Drilling Rig #72. KOP at 5350 ft. on second sidetrack. EOB at 8344 ft. MD (7870 ft. TVD). Currently at 16,746 ft. and drill ahead.
