Present at time of inspection was Coby Muller (Range drilling consultant). The site is located in West Finley Twp., Washington County. The inspection was a routine inspection related to a report of possible impact to water well from vehicle accident on site.

A report was received that a unmanned vehicle had drifted over a hillside on the well site and had impacted a private water well. The vehicle had drifted off the access road and struck an unused concrete watering trough. The trough was not damaged.

The operator is in process of setting well pad secondary containment for a rig move onto site to complete drilling operations on the five wells on site. The access road and well pad were in good condition. Accelerated erosion was noted in the well pad Retention Basins and collection channels for those basins. 2625937 Primary Facility 2017-08-16 Incident- Response to Accident or Event No Violations Noted On August 15, 2017, the Department performed an inspection of the Range Resources Gene Sheller well pad site. The site is located in West Finley Twp., Washington County. The inspection was an inspection related to a reported spill of glycol to the well pad by the operator.

Derek Zofchak (Range Respurces) reported to this inspector by phone on 8/11/2017 at 12:36 PM, that at approximately 12:14 PM, approximately 7-10 gallons of diethylene glycol released to the graveled well pad during the filling process of the 3H GPU. While the GPU was being filled, no fluid was shown in the sight glass which is an indicator of the current fill level. Shortly after, glycol began to burp from the bath expansion boot vent that is located on top of the GPU. The release was noticed immediately and the driver shut the fill nozzle thus stopping the release. At this time, an employee also noticed a small diethylene glycol leak from the gasket located on the back of the GPU where the fire tube is bolted to the diethylene glycol b

The inspection was an routine inspection during installation of the production equipment on site in preparation for hydraulic fracturing operations.

The site is located in West Finley Twp., Washington County. The inspection was a routine inspection of the site after a request by the operator for a restoration extension.

The Act 9 sign was in place and legible.

The access road and well pad are in good condition.

The site is has greater than 70% perennial vegetative cover.

5 wells have been drilled to completion on site and are equipped for production.

Given the condition of the site at time of inspection, the site restoration is granted until February 15, 2020.

The operator has requested a minor modification to the permit to extend the well pad drainage channel to retention basin#1. The minor modification was reviewed and approved in the field.

5 wells have been drilled to completion on site and are equipped for production.

The GPUs for the 5 wells equipped for production are in good condition, but are not in secondary containment as required by § 78a.64a(b). The operator will need to place the GPUs into secondary containment.

2779857 Primary Facility 2018-09-20 Routine/Complete Inspection No Violations Noted On September 20, 2018, the Department performed an inspection of the Range Resources Gene Sheller well pad site. The site is located in West Finley Twp., Washington County. The inspection was a routine inspection of the site after a report by the operator of slope failure on the site.

The Act 9 sign was in place and legible.

The access road and well pad are in good condition.

The operator has notified the Department that a cut slope in the Retention basin (PCSM)on the south side of the well pad has failed a second time. The operator repaired the slope failure in 8/18 and the area has failed a second time. The operator will need to complete a geotechnical investigation and report for repair of the slide and stability of the site. The collection channel for this same retention basin (PCSM) has developed some accelerated erosion with recent heavy rainfall. The operator will need to address the stability issue with channel.
