Moved onto this well on 5/8/17 and resumed drilling at 53 ft. KB. The 18 5/8” casing was set at 653 ft. on 5/9/17 and cemented to surface with 65 bbls returned. Currently at 847 ft. and drilling ahead.

2596213 Primary Facility 2017-05-19 Drilling/Alteration No Violations Noted Fountain Quail Energy Services Rig #6

The 13 3/8” casing was set at 932 ft. on 5/10/17 and cemented to surface with 57 bbls returned. Coal was found from 887 ft. to 892 ft. A basket was set at 847 ft. The 9 5/8” casing was set at 2582 ft. on 5/13/17 and cemented to surface with 40 bbls returned. A Casing Annulus Packer was set in the 9 5/8” casing string from 1784 ft. to 1794 ft. KOP was at 4875 ft. TD was at 6610 ft. MD on 5/16/17.

2705034 Primary Facility 2018-03-07 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Cogar well pad and the following was observed.

Proper signs were displayed identifying the site. EQT currently has a rig on site and is drilling 1 of 6 new wells. The well heads onsite are contained inside subsurface vaults covered with equipment for the drilling process and could not be visually inspected. The production tanks were in good condition no leaks , rust or defects observed. Secondary containment for the site was intact and maintained.

The site had some stabilization issues on the SW side of the access road. I there was some slides forming inside and outside the LOD. I spoke with Brian Morris from EQT about the issue and we arranged a site inspection on 3/13/18 at 1:30pm to discuss the plan for stabilizing that area.

2710035 Primary Facility 2018-03-26 Drilling/Alteration No Violations Noted Pioneer Drilling Rig #72 moved onto this well on 3/9/18 and resumed drilling at 6610 ft. MD KB. KOP was at 4895 ft. (with air rig). EOB was at 8629 ft. MD (7580 ft. TVD). TD was at 16,793 ft. MD (7676 ft. TVD) on 3/15/18. The 5 1/2" casing was set at 16,787 ft. Estimated TOC at approximately 4623 ft.
