2593327 Primary Facility 2017-05-15 Routine/Complete Inspection No Violations Noted On Monday, May 5, 2017, I completed a routine-complete inspection at the Cashdollar Well Site located in Amwell Township, Washington County.

The Act 9 sign was in place and clearly legible at the site entrance which is secured with a gate and a lock. The access road and well pad are in good condition and the site is stabilized with vegetative growth. The operator may consider re-evaluating the vegetation on-site to determine the necessity of keeping the filter sock and silt fence in place or whether it can be removed. The operator should also continue their periodic monitoring program and address any deficiencies, signs of accelerated erosion and/or sedimentation upon identification.

The secondary containment structure around the production tanks appear to be in good condition and functioning as intended. The gas wells appear to be in good working order and are labeled as required.

No violations were noted during the inspection.

2628019 Primary Facility 2017-08-14 Drilling/Alteration No Violations Noted Cellar Tech auger rig

Drilled a 30” wellbore and set 26” conductor casing at 50 ft. on 7/31/17.

2628026 Primary Facility 2017-08-14 Drilling/Alteration No Violations Noted Fountain Quail Energy Services Rig #6

Moved onto this well on 8/8/17 and resumed drilling at 62 ft. KB. The 13 3/8” casing was set at 862 ft. on 8/9/17 and cemented to surface with 40 bbls returned. No coal was observed. No basket was set. The 9 5/8” casing was set at 2117 ft. on 8/11/17 and cemented to surface with 45 bbls returned. KOP was at 3550 ft. Currently at 5660 ft. MD and drilling ahead.

NOTE: There was no 18 5/8” water-protective casing set on this well.

2821192 Primary Facility 2019-01-02 Routine/Complete Inspection Violation(s) Noted Today I conducted a routine inspection at the Cash Dollar well pad and the following was observed.

Proper signs were displayed identifying the site. The access road was maintained and in good condition. The outer slopes were stabilized with thick vegetation no erosion observed outside of the LOD. The well heads were maintained no leaks or defects observed. The production tanks were in good condition no defects or leaks.The production tanks secondary containment was intact.


Note: Wells 125-28214 and 125- 28156 violations for no secondary containment under the GPU's 78a.64a(a). The other wells on the pad were spudded and drilled prior to the 2016 regulation and are exempt of 78a.64a(a).
