Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
1441915329 2017-04-03 Production Wells No Violations The purpose of my inspection was to document the correction of well identification on well #2-1408. The identification sign at the well head has been changed to reflect the correct permitted information. The identification sign that was present on the separator door was removed. No further action is necessary.
1729474175 2017-03-16 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1408. A review of RBDMS indicates the Owner is current with reporting production. Lease access was through a residential driveway. The well location was partially snow covered. The location was stable. The identification sign reported the incorrect permit number, county and township. The 8.625 well head was below grade. The 4.5 production casing was equipped with a plunger lift assembly. The Demco gate valve was in the closed position. No audible or visual leaks were observed. A domestic supply connection was present. An earthen dike stabilized with well-established vegetation was present around the production equipment. Equipment within the dike consisted of one (1) horizontal separator, one (1) 50 bbl production tank, and one (1) 5 bbl poly brine tank. The identification sign on the separator door reported the incorrect permit number, county and township. The separator remained in good repair. No audible or visual leaks were observed. The meter house did not contain a current production chart. The production tank was in good repair. No holes or leaks were observed. The load line valve was closed and plugged. The drain line valve was closed and plugged to a 5 bbl poly brine tank. The hatch lid was closed. The poly brine tank was in good repair. No cracks or leaks were observed. The inlet valve was closed and locked. While on location I called and spoke with George Strawn concerning the incorrect identification signs. Mr. Strawn stated he would contact his son who operated the well to correct the identification within a week. I will re-inspect the to confirm the correction has been made.
