The new owner has corrected well identification present at the entrance and well head. Lease access and the well pad facilities remain in good repair. No other equipment is on the pad. The Owner raised and identified the 9.625 intermediate annular casing pressure gauge and the 5.5 production annular casing pressure gauge. The intermediate annular casing gauge indicated 0 psi. The production casing annular gauge indicated 160 psi. The 9.625 intermediate casing seat is set at 2870 Maximum allowable annular surface pressure (MAASP) at the surface per ORC 1501:9-1-08 (N) (2) is 869 psi. The production casing valve was closed. The production casing and tubing pressure gauges indicated 0 psi. I will continue to monitor casing pressures on a regular basis to ensure compliance. |

The access road remained in good repair. No evidence of accelerated erosion or sedimentation was present along the road. The pad was enclosed by a locked chain-link fence perimeter. The fence remained in good repair with no signs of vandalism. An earthen berm perimeter was present outside of the fencing. I walked the perimeter of the pad and found no evidence of erosion or sedimentation. The pad was composed of crushed gravel that remained in good repair. A jersey barrier perimeter was present around the well head. Correct signage was present on the well head. Annular pressure gauges were identified. The 13.375 annular gauge identified 0 psi. The 9.625 annular gauge identified 155 psi. The 5.5 production casing gauge identified 0 psi. The tubing gauge identified 0 psi. No audible or visual leaks were detected. The second year inactive status permit issued to Chesapeake on 6/3/2016 expired 6/3/2017. At the time of inspection a valid third year temporary inactive status permit has not been issued. The well has no reported production for eight consecutive reporting periods as reported in accordance with section 1509.11. The Owner shall plug the well in accordance with section 1509.12 of the Revised Code, obtain temporary inactive well status in accordance 1509.062, or perform another activity regarding the well that is approved by the chief of the division of oil and gas resources management. |

The current temporary inactive status permit was issued on 8/14/2017. At the time of my inspection the identification sign at the entrance was lying on the ground. Access to the well pad remained stable and drivable. Mature vegetation was established in the erosion and sedimentation controls. The pad perimeter security fence remained in good repair. The cut and fill slopes remained stable. Correct identification remained in place at the well head. A perimeter of jersey barriers were in place around the well head. Steel grating remained in place over the cellar to prevent falls. The cellar contained freshwater. I did not witness any evidence of leaking. The annular gauges were plumbed above grade and labeled. The intermediate casing annular gauge indicated 0 psi. The production casing annular gauge indicated 150 psi. The well head was equipped with a production tree. All valves remained closed. A chain was placed through all of the valves and locked. The production casing annular gauge indicated 150 psi. The production casing gauge indicated 650 psi. The tubing gauge on top of the production tree indicated 4100 psi. No other equipment was on location. While on location I called the Geopetro office to report the condition of the entrance sign and to request it be repaired. I will schedule a follow-up inspection to document the status of the sign. |

There is one well on the pad. The sign board posted at the lease entrance displayed signs with the well name, address, permit number and emergency contact information. The lease road was graveled and in good repair with a gate at Vale Road. Erosion and sedimentation controls were in place and ground cover was well established along the road. I walked the entire location and found the well pad constructed with an earthen berm perimeter in good repair, being stabilized with well-established vegetation. Erosion and sedimentation controls are in place and functioning. The well pad was found to be enclosed by a chain link fence with barbed wire at the top. The primary entrance and escape doors were found locked. The surface of the well pad consists of gravel and is in good repair. Soil stockpile is stabilized with well-established vegetation. The Carney 3H was constructed with 120 of 20 conductor, 835 of 13 3/8 surface casing, 2869 of 9 5/8 intermediate casing and 13596 of 5 1/2 production casing. Drilling was completed 5/5/2012. The well was stimulated 7/20/2012 and completed with 8183 of 2 7/8 tubing. The well head was constructed with a Christmas tree valve assembly and found to be protected by steel cattle gates further protected by concrete Jersey barriers. Proper well identification signage was displayed. A cellar grate was in place to prevent falls. The cellar contained water with approximately 5 feet of freeboard. The pressure gaugse on the production casing, intermediate casing, and surface casing all indicated 0 psi. No sheen or leaking gas was observed on the water in the cellar. The mousehole was cemented. Gate valves are chained and locked. No other production equipment is on the pad. I recommend that the pressures associated with the respective casing strings be tested and documented with an ODNR DOGRM Inspector present as a |

At the time of my inspection, a mature tree was positioned across the entrance that prevented access to the well location. The entrance sign remained on the ground. I walked back to the well pad and found the perimeter security fence in good repair. I did not witness any other signs of vandalism. While on location I called the Geopetro office and spoke with the receptionist. I stated the reason for my call and indicated that I would be issuing a compliance notice due to the sign not being repaired after my previous inspection and phone call on 9/19/2018 (Inspection # 1557401513). The receptionist stated that she would have the field superintendent call back to discuss the situation. I will schedule a follow-up inspection to document the status of repairs. |

I had spoke with owner representative Andy Iezzi on 11/15/2018. Mr. Iezzi reported that the entrance sign was repaired and that he had updated emergency phone number stickers that would be installed when weather conditions were favorable. He also reported that the land owner had placed the tree across the entrance road due to trespassers. At the time of my inspection I found the entrance sign standing. The emergency phone number was not updated. I met with the surface owner, Mr. Carney, who confirmed trespassers had been leaving trash on the road at various times and had likely vandalized the entrance sign. I will follow-up with Mr. Iezzi in a few weeks to confirm the emergency number is updated. No further action is necessary at this time. |
