0224943906 2017-10-13 Production Wells No Violations The purpose for my inspection is due to a Compliance Notice Follow-up to number # 1470073012. Porter Petroleum had attempted to clean the site on 4-4-17. On that date the Division Inspector Shreve was on site taking field samples as the company contractor Andy Browning was in with equipment cleaning up the brine saturated soils. Since that time the company had come in to the site and replaced a steel 100 barrel tank and replaced with a 1100 gallon plastic tank. There were still brine scalds down over the dike wall and within the dike. Brent Bear was requested to come out to verify the site. The field samples were taken and showed that the Chlorides were at 3993 Parts per million within the dike and 1700 parts per million down over the hill. I will get with the company and revise the plan for the clean-up.
0305020718 2017-04-04 Production Wells No Violations The Inspection is due to a follow-up to Compliance Notice # 1470073012. On the day of my inspection Andy Browning the contractor working for Porter Petroleum had equipment in to complete the clean-up of the brine scald at the bottom of the tanks. They brought in a load of topsoil to replace the dirt that was taken out. Andy took some scoops of soil out where the grass was not growing and I sampled the soil for chlorides with the low Quantabs reading 150 ppm. There was a small coal seam that was letting the brine seep through and that was removed. The chlorides were down to 31ppm. The topsoil was spread out and compacted. The crew then seeded and mulched. There was approximately 3 ton removed.

0858757542 2016-10-06 Production Wells No Violations The purpose of the inspection is a Compliance Notice Follow---100 barrel steel tanks have somewhat of a dike. The area from the brine in soil is still present and has not been removed and replaced. Company has been instructed by two Regional Supervisors to get the issues remedied after their Inspection with the Company on 8-16-16. I took a sample of the soil and performed a field test for the Chlorides. The sample verified that the Chlorides were at 1700-1800 Parts per Million.
1173111881 2017-01-09 Production Wells No Violations The purpose of the inspection is due to a Compliance Notice Follow-up to number 1470073012. Brent Bear and I inspector Patrick Shreve met with Darrel Porter and Andy Browning at the tank site. We discussed the violations that are still pending at the well and the tank site. Browning would be in no later than 5-1-17 to remedy the issues with the well and the tank site. There will be a bull plug installed at the well head. The elevated chloride soils will be pulled out from below the lower tank and hauled away. New top soil will be brought in to replace the missing soil. Then all will be graded, reseeded and mulched back.

The production equipment is enclosed within a earthen impoundment. The identification is posted at the well and the tank. There is good vegetation and more coming. The Compliance Notice is resolved.
1440244486 2016-10-19 Administrative Inspection No Violations I called Virginia Porter on what I need to be completed. Place a Bull plug where the cam lock fitting is on the well head. Then remove brine saturated soils and replace with clean soil. Then seed and mulch the area. I stated that I could be there to sample so a lot would not have to be removed.

1472220943 2016-08-16 Production Wells No Violations An inspection of the well and related tank battery was conducted with SW Regional Supervisor, Dan Goins and well owner, Daryl Porter. Mr. Porter had requested an inspection after receiving a Compliance Notice from Inspector, Pat Shreve on this well.

Mr. Porter agreed to complete this work within two weeks from today and contact Pat Shreve to be present for the cleanup. Approval was granted to remove soil from the property, remediate offsite at a non-related property, and document how and when treatment was applied to the soil.

1480784142 2016-10-03 Production Wells No Violations The purpose of the inspection is due to a Compliance Notice follow---100 barrel steel tanks have somewhat of a dike. The area from the brine in soil is still present and has not been removed and replaced. Company has been instructed by two Regional Supervisors to get the issues remedied after their Inspection with the Company on 8-16-16. I called the Company and talked with Virginia Porter. She stated Darrel has been busy with work and just explain what I need to do to be in Compliance. I stated that I need the Cam- Lock fitting removed and install a steel bull plug at the wellhead on the tubing annulus. The Brine scald below the tanks needs removed and replaced. It would be easier if I was at the site to test the soil as it is being removed. That way there would be minimal removed. She would let Darrel know. Pictures were taken on the day of my inspection.
1482329913 2016-10-19 Production Wells No Violations The purpose of the inspection is due to a Follow---100 barrel steel tank batteries that are enclosed with a dike. The lower tank has the deepest portion of the impoundment. There is a scald area below the lowest tank that still needs to be remediated. I will call Porter Petroleum on the issues.
1669170079 2016-12-28 Production Wells No Violations The Inspection is due to a follow-up to Compliance Notice # 1470073012. I stopped by and talked to the land owner at the home. Her name was Molly Norris. She stated that she wants company to clean up their messes. If it is their responsibility then they should clean it up. There is no domestic gas hook up. There is a propane tank across road from the house which goes to the address 4605 Pert Hill road.

Porter Petroleum had attempted to clean the site on 4-4-17. The inspection revealed the well is equipped with and Allied pumping unit, electric motor, tubing head and 2 3/8-inch tubing. The tubing and casing valves are open. The tank site consists of [1] 1200-gallon plastic tank and [1] 100-barrel steel tank. All the equipment is enclosed within an earthen impoundment. Identification is not posted at the tanks. There is a small area below the tanks that has not vegetated back. I will give some more time and check back to see if the vegetation takes off.
