At the time of this inspection, I observed the following: there had been no notable changes to the wellhead. The wellhead with 8.625-inch surface casing with valve tree remained extant with no observable changes. The flow-line connections remained disconnected and not capable of being produced. The area around the wellhead was significantly overgrown with small trees and other dense vegetation with no discernible access road. There was a fenced enclosure that was the inferred former location of a tank battery system but which had become overgrown with vegetation with no production elements remaining.
1444859604 2015-10-14 UrbanProduction Wells No Violations I inspected this well as a routine inspection. The well is located on former industrial property which is now dormant. The well is in a wooded portion along the eastern banks of the Grand River. At the time of this inspection, the well is comprised of a 8.625-inch surface casing with 4.5-inch production casing with a 4.5 to 2 reducer swage and closure gate valve which is closed. The 2-inch flow-line connection is disconnected. The area surrounding the well is heavily wooded and the wellhead is covered and overgrown with grapevine and other vegetation. There are no aboveground production tanks or other appurtenances apparent. There has been no production reported since 1999 and no annual production reports filed since 2009. The well owner/operator shall submit a written plugging schedule to Inspector Stephen Tompkins at 3575 Forest Lake Drive., Suite 150, Uniontown, OH, 44685 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

At the time of this inspection, there has been no chanage at the wellhead or production reported. Flow connections were disconnected. Elements at the welllhead consist of a 4.5 to 2 reduction swage to a 2 flow line lateral which was disconnected. The well was heavily overgrown with vegetation.
