0407659979 | 2014-11-20 | UrbanProduction Wells | No Violations | I inspected this well location as part of routine inspection activity. I observed the pump jack and tank battery to be operational and free from signs of leaks or other problems. Secondary containment is intact. Load-outs are bull-plugged. The enclosure gates are locked. ID signage is present and accurate. The short, earthen-paved access road to the tank battery and wellhead is in good condition. |
0488306127 | 2017-06-08 | Production Wells | No Violations | I inspected this well as part of routine inspection activity. The well with pump-jack system and associated tank battery located on a larger, otherise undeveloped parcel, most of which consisted of woods and meadow with mature, wild vegetation. The tank battery is shared with the Middletown Bank #1 (#2-0546). The fittings and connecitons at the wellhead and battery were secure. I walked the areas around the well and battery and found so indications or signs of problems. I noted that the signage had become faded and notified Ron Gibson about the need for maintenance. Production reports were current and there were no indications of deficiencies with bonding or insurance in a review of electronic file information conducted on this date. |

1139069564 | 2012-04-25 | UrbanProduction Wells | No Violations | My findings for this inspection; The tank battery has two 100 barrel production tanks and a mechincal separator in it. The tank battery dike is in good repair. The well has a pump jack unit on it. The operator has fenced the pump unit. The lease road is stable and constructed out of durable material. The operator has bull plugs in the valves on the bottom of the production tanks. The well needs an identification sign. Operator has recently worked over the well and replaced the west production tank. The stuffing box on the new pump unit has a minor leak. Theres a approximately a quart of free oil on the casing and 2 to 3 quarts of oil staining the gravel around the wellhead. Operator will be contacted in regards to the leak and the identification sign. |

1999993647 | 2013-10-02 | UrbanProduction Wells | No Violations | The producing unit consists of a pump jack unit at the wellhead and an aboveground storage tank (AST) battery. The battery is comprised of 2 100-barrel steel production tanks and a separator unit. The elements are enclosed in separate chain-link enclosures. There is adequate secondary containment and signage at the wellhead. No problems noted. |
