Upon inspection , it was observed that this well is equipped for gas production only. The well consists of a wooden conductor (mostly rotted away) and 6.625 casing with a gas escape packer with 2 production tubing and rods. Water could be seen approx. 6 down inside the wooden conductor hole. The 2 line that was attached to the production tubing is broken at the fitting. There is a 1 poly line attached to the 1 gas escape tubing feeding into a nearby gathering line. The scissor jack pumping unit has been removed from the well and the polish rod is very corroded . No pollution or contamination issues were observed. There is no identification information posted at the well or production tank. According to RBDMS production records, this well produced 170 mcf of gas in 2012. No production records were submitted for 2013. P&D Valentine Oil Co. will have 14 days upon receipt of this notice to submit current production values and post I.D. at the well location. |

Since the records have yet to show up on RBDMS, I instructed Mr. Valentine to mail the records to the ODNR DOGRM Zanesville Office so they can be scanned into the system. The compliance notice for this well can be closed out. |
