0540750421 2008-06-24 Drill / Deepen / Reopen No Violations Inspection revealed that G & L Drilling filled the conductor hole full of cement and are trying to drill the cement at this time. They had a big rock that kept falling into the wellbore so they filled the hole with cement in an attempt to cement the rock in place so they could drill it out.
0558944275 2019-01-23 Plug / Plug Back No Violations Upon my arrival on the snow- covered location, I witnessed the rig loading the vac truck with fresh water. They were stringing out water hoses and cement hoses to set a cement plug in the well. They pumped a 200 foot (37 sks) Class A cement plug from 470 to 270 feet. They displaced cement with 4.8 barrels of fresh water. The rig crew started to pull the 4.5 inch diameter casing out of the well and it would not move. With the approval of ODNR District Supervisor, Mike Moreland, the rig crew shot the 4.5 inch diameter casing off at the depth of 203 feet. This left about 267 feet of 4.5 inch diameter casing in the well. The cement plug should range from 470 feet to 242 feet, with 4.5 inch diameter casing in it. Then there is 39 feet of 4.5 inch diameter casing on top, with no cement in it. The rig crew then pulled the remaining 203 feet of 4.5 inch casing up 3 feet and set another cement plug. They pumped a 200 foot (38 sks), Class A cement plug from 200 feet to ground level. The rig crew pulled 5 joints of 4.5 inch casing out of the well and topped off the well with remaining cement. The crew is going to rig down and move to another well to plug. They will then cut the 7 inch diameter casing off and reclaim both wells at the same time.

0907710016 2008-07-14 Completion Testing No Violations Inspection revealed that the 4 1/2 casing got hung at app. 750 . The service rig ran tubing and are swabbing thru the tubing at this time. No violations at this time.
0963080467 2008-06-30 Drill / Deepen / Reopen No Violations Inspection revealed that G & L Drilling has TDed the well . Inspectiuon revealed no violations at this time. The crew advised that they didnt run 4 1/2 casing because the well was gasing and the operator didnt want to run casing while the well was gasing. The operator is going to run the production casing with a service rig.
1199592965 2016-11-23 Production Wells No Violations On the day of my inspection I witnessed the permitted well bore is utilizing a pump jack for means of production. The well bore consists of 8.625 inch diameter surface casing, 4.5 inch diameter production casing and 1.5 inch diameter production tubing. At the time of my inspection the pumping unit did not have a prime move in place. All production valves were placed in the open position. Production equipment for the well bore consisted of a 100 barrel production tank and a 2 inch diameter meter run. The meter run showed no indications of recent use. The 100 barrel production tank was placed in an earthen dike that measured approximately 20*20 feet at an average height of 3 feet. RBDMS records indicate that the well has not produced commercially since 2012. I will perform a follow-up inspection to assure that the well is still idle before writing a compliance notice to the well owner.
1219859728 2017-10-31 Production Wells No Violations On the day on my inspection I found this well equipped with a walking beam pumping unit. This inspection is a follow up to compliance notice #2105427973. Well is equipped with 8.625 inch diameter casing, 4.5 inch diameter casing, 2.375 inch diameter tubing, and rods. There is no motor on the pumping unit. Tubing valve is open, casing valves are shut. Production equipment includes a 100 barrel steel tank inside of containment. The tank has a bullet hole in the side of it. RBDMS indicates well was last produced in 2011.
1312149262 2019-01-09 Plug / Plug Back No Violations On the day of my inspection I witnessed Unlimited was tripping 2.375 tubing from well. They pulled the rods yesterday and finished tripping tubing during my inspection. Their tally indicates they pulled 1741.55 feet of tubing. They are going to verify it is free of obstructions and trip it back in to set bottom plug tomorrow.

The tank has a bullet hole in the side of it. RBDMS indicates well was last produced in 2011. I have been emailing with Mark Obrien who is supposed to be submitting plugging permits for these wells on behalf of the deceased owners wife. His last email dated 1/2/18 says I should receive them this week.

1557930806 2019-05-13 Plug / Plug Back No Violations On the day of my inspection I witnessed this well has been plugged. Mark Obrien had contacted me in regards to outstanding reclamation on this well. I spoke with Gary Lang, who is contractor responsible for the reclamation, to convey what was needed and that the weather is not conducive for reclamation this week. Currently the wellhead needs cut off and buried. Cement is near surface level. Small pit needs closed and tubing needs removed from site. Minor grading work and everything affected will need seeded. Landowner waiver for lease road will be necessary.

They pulled 50k and were unable to pull the packer. They worked it up and down for a while. They will shoot it off tomorrow and their final plugs then.
