1281370700 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are utilizing a plunger lift system. RBDMS production records indicate that the wells are in producing order.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 6 Producing wells 6 Sand separators

2 Horizontal heater-treaters 1 combustor 1 Low pressure separator tower/Vapor recovery system unit. 1 compressor The site passed preliminary restoration in accordance with ORC 1509.072 A.

1363825351 2013-03-20 Drill / Deepen / Reopen No Violations I am on site today to witness the running and the cementing of the 13-3/8inch diameter casing string. Nomac Drilling Rig # 71 drilled a 17-5/8 diameter hole to 615 ft and ran 602 ft of casing. Baker Hughes Cementing Service c/w 556 sks of class A cement . During the inspection it was determined that they would not get casing to bottom for several hours. Returned that evening after contacting Uniontown to advise and witnessed cement circulate to surface. After cement was displaced and the plug was landed the cement head blew off at 550 P.S.I. The floats held and no cement flowed. No one was on the floor at that time so nobody was hurt.
