1392397066 2014-02-14 Casing String Witnessed No Violations On 02/13/14 1308 of 7 surface casing was set and cemented to surface using 275 sacks of class A 3% calcium-1/4# flake cement with 50% excess ran and 20 barrels of cement returns to surface, the well will now be turned over to the region UIC inspector Dave Ball for the production borehole and casing completion.

Note: Total depth of well was noted at 8005 and they ran a total of 7938 of 4.5 casing and Baker Hughes cemented it in place with 826 sacks total. Note stage tool@4112 and they had good returns on both stages of cementing process.

1394378958 2014-03-09 Production Wells No Violations On site to witness Baker Hughes try and perform squeeze job on stage tool, bridge plug was set at 4165 and retainer was set at 4105 and stage tool was@4112 GL. They pumped water ahead and then stung into retainer and hole pressured up to 3500# and after several attempts they decided too drill out retainer and pressure test casing again.
1394485568 2014-03-10 Production Wells No Violations On site to witness Baker Hughes try and perform squeeze job on stage tool, bridge plug was set at 4165 and retainer was set at 4105 and stage tool was@4112 GL. Baker Hughes on site again too squeeze cement on faulty stage tool.

Note: Baker Hughes used 100 sacks of cement to perform squeeze job.

1394655675 2014-03-12 Production Wells No Violations Squeeze job on stage tool failed again and decision was made to acquire a casing patch and place it over the faulty stage tool.
1395706621 2014-03-24 Completion Testing No Violations Status check inspection on the 4.5 11.6# production casing repair process, the company has a work over rig on the hole and has set a Weathorford casing patch over the 2 stage DV tool at 4011 after the initial casing pressure test did not pass and indicated the DV tool was leaking, the initial 20 casing patch did not pressure test after being set and was milled out and the company has run several casing logs and is preparing to set a second patch to be 40 long, the casing patch burst pressure is rated at 9,000 psi.
1396197200 2014-03-28 Production Wells No Violations On site to witness Archer Wireline Company perforate casing at 7545-7551 24 shots and once perforation procedure was complete it was noted that wellbore went on slight vacuum and fluid levels in hole were down 1000 when they brought out wireline tools.

1410305578 2014-09-09 Salt Water Injection Well No Violations Inspection revealed that there has been no change in status of well it still needs to be completed and tubing and packer ran into well and surface facility constructed.

Weatherford inflatable packer was run to a depth of 7477 and packer deployed and they tried to pressure test after pumping 106 barrel of fresh water and inhibitor and testing failed they will try and set dart inside of tubing to pressure test it and if it checks out they will pull entire string from well bore.

1412026448 2014-09-28 Salt Water Injection Well No Violations On site to witness tubing and packer run into well bore this is the third Weatherford Inflatable packer that has been run into this well and setting depth is 7457 and we tested annulus at 400 psi with no decline in pressure.

Note: Two pen recorder required to monitor pressures when owner get authorization to place well into injection well status.

1441068900 2014-10-27 Completion Testing No Violations On site to witness injection test on said well I monitored injection pressures to make sure they did not exceed maximum injection pressure of 1935 psi and also monitored annulus pressure to make sure injection string or packer showed any signs of loss integrity.

Note: Testing was cancelled after six hours of fighting pressure issues.
