Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0400345126 2018-05-03 Casing String Witnessed No Violations On site inspection to witness the witness the conductor casing cement job. Precision Rat hole is on location along with Stingray Cement Service. The Auger type rig spudded the well on 05/02/2018 and drilled a 30 diameter conductor well bore to a measured depth of 120 and inserted 120 of 24 diameter 94 pound per foot weld connection casing into the well bore. Stingray Cement Service cemented the casing using a 62.5 barrel absolute volume cement mix consisting of 290 sacks of class A cement with the cement weight verified at 15.6 pound per gallon by weight of scale, the cement yield was 1.21. The cement additives was 2% Calcium and .25 pound per sack celluseal. The cement was displaced to 110 with 57.7 barrels of fresh water. Cement returns were witnessed to surface inside the tin horn cellar. 3 conductors were set and cemented today. The job was called out for 14:00 but did not start until 18:30. The company has also started to lay containment and move the drilling rig in while conductor operations are in progress.
0990639821 2018-11-13 Preliminary Restoration No Violations Preliminary restoration status check inspection found the well pad surface and lease road is of stone construction and in good repair. There is vegetation established on all outward facing slopes of the well pad and erosion control in place. The well pad perimeter drains are in the closed position. There is disturbance on the outward slopes where the 3rd stream party pipe line has been installed and will need more vegetation growth before the restoration can be considered complete. The perimeter berm on the South West corner of the well pad has been removed and not sufficiently restored to proper heigth. The location was in close proximity to the tank battery and found at 39.65569 x 80.90328. Bruce Carpenter with Eclipse was notified at 11:45 am and asked to address the issue with the berm. There are 3 wells on the pad in production. The said well has a manual master valve installed on the B section of the well head. The 5.5 production casing internal working pressure as indicated on the well head gauge is 7,139 pounds per square inch (psi) there are (2) production flow lines attached and associated with this well at the well head with 2,700 psi on the production flow lines down stream from the well head pressure reduction choke system. Attached to well head A&B sections are riser pipes with valves and gauges present to monitor annular pressure, the riser pipes are equipped with gauges. The A section gauge shows zero psi, this is the area which is between the 9.625 and 13.375 diameter casing strings. The B section gauge shows 600 pounds pressure, this is the section between the 9.625 and 5.5 diameter casing strings. The tank battery consists of (4) steel construction 400 barrel capacity tanks that are situated inside a polymer lined steel containment dike area. There is a low pressure vapor recovery system present associated with the production equipment. There is multiple line and glycol heaters present on location.
1061873064 2019-01-28 Production Wells No Violations On site inspection found (3) wells on the pad that are in production. The said well is producing off the 5.5 diameter production casing through a primary master valve and into (2) sales lines, there is also a single line heater for each flow line. Down stream from the line heaters is (2) gas processing units. Annular pressure monitoring is in place with 1,350 pounds pressure on the B well head section and zero pressure noted on both the A section, the well tender on site was advised to address the 1,350 pound pressure on the B section. The tank battery is at the right rear corner of the well pad and consists of (4) steel tanks capable of holding 400 barrels fluid each, the tanks are situated inside a polymer lined steel containment dike area. Identification is posted at the well heads. The lease road and well pad surface are stone construction and in good repair. Also noted was the company was stacking out drill pipe on the location and a 3rd party contractor identified as Fairmont Tool was on site certifying the pipe. The pipe was being trucked in by Grant Smith Trucking and being unloaded by Fairmont Tool Employees within 10 of the (3) live well heads. The truck was being parked parallel to the wells. I notified the Fairmont Tool Employees along with Eclipse Personnel and asked them to move back a safe distance from the well heads while unloading pipe.
1531167390 2018-07-09 Casing String Witnessed No Violations On site inspection to witness the surface casing cement job with H&P Drilling rig 608 on location along with Schulumberger Cement Service. The rig has drilled a 17.5 surface borehole to a measured depth of 1,985 from a drilling rig Kelly Bushing height of 26.8 above ground level elevation. The rig and Schlumberger attempted to circulate the last casing joint in the hole unsuccessfully to set depth of 1,841. The well head was a FMC mandrill type and the casing was 5 from landing out in the mandrill so the decision was made to cement the casing in place 5 short of landing then switch the well head out to a slip style well head. Thus delaying the job 6 hours. The cement job consisting of 1,525 sacks of class A cement with a 1.18 yield and 15.6 pound per gallon weight verified by weight of scale. The absolute volume cement mix was 320 barrels equaling 30 % open hole cement excess pumped. The cement was displaced to the float collar set at 1,792 with 276 barrels of fresh water. Due to the extended time on location I left location at 21:00 as the cement job was being started.
1532360401 2018-07-23 Drill / Deepen / Reopen No Violations
1535569290 2018-08-29 Completion Testing No Violations Status check inspection on the well completion process found Halliburton pressure pumping service along with Nine Energy wireline service. The 5.5 production casing was set to a measured depth of 23,170. The well will be stimulated in 60 stages total. Halliburton recently completed stage 12. The average treating pressure was 10,962 pounds per square inch (psi), the initial shut in pressure was 7,600 psi. The total fluid volume used was 9,400 barrels. The well head master valves and flow cross is rented from FMC. The B section annular pressure is being monitored electronically with 14 psi showing on the electronic transducer between the 5.5and 9.625 diameter casing strings. A manual gauge is on the A section and is being recorded daily.
1997493884 2018-07-16 Drill / Deepen / Reopen No Violations On this day, I found the H&P Flexrig 608 conducting a rig inspection after cementing the 9.625 inch diameter casing at a depth a 8,852 feet. They will continue with drilling the 8.75 inch diameter borehole after the inspection is completed. The projected kick-off point is at a measured depth of 10,306 feet, and the landing point is projected at a measured depth of 11,266 feet.
