A police report was filed. A small percentage of the release seeped through the western dike wall and affected an area 10 feet by 15 feet. The initial response included spreading sorbent materials outside of the dike and using Moore Well Services vac truck inside of the dike. Recovered oil was placed back into a stock tank for later recovery. The Christ United Methodist #1-D well has a common tank battery with the St. Sebastian #1, #2-D and #3-D wells. Future reports will on reference the St. Sebastian #1 well. If remediation continues in a timely fashion, no NOV will be issued. All valves will be locked. |
1447635101 | 2018-05-16 | UrbanProduction Wells | Violations Noted | The well was produced as a plunger lift well. It was surrounded with a locked fence. Legible I. D. signage was posted. A domestic house connection was attached to the wellhead. No leaks or contamination were found at the wellhead. The tank was located at the ST. Sebastian Church tank battery in the gym parking lot. The Christ Methodist Church # 1 shares a common tank battery with the St. Sebastian # 1, # 2-D and # 3. The tank battery was fenced and locked. I. D. signage was posted. The load lines were plugged and locked. The 140 barrel stock tank and a 2408 horizontal separator made up the production equipment for the well. The 140 barrel tank ( # 35379 ) had holes which allowed the contents to leak and cause ground contamination. I spoke with George Strawn ( Everflow Eastern ) and inform him of the situation. He said, he will assess the situation tomorrow ( 5/17). There are multiple tanks in this tank battery that are in the same condition and will require replacement. In order to bring the Christ Methodist Church # 1 well into compliance Everflow Eastern Partners LLC shall remove the faulty tank, excavate any contamination and replace the tank with one that has integrity. Everflow Eastern shall contact Bob Roush ( 330.690.8837 ) in order that Mr. Roush assess the compliance with OAC 1501:9-9-05 A -1 and OAC 1501:9-1-07 (A). This well shall be brought into compliance by 6/1/2018. |
