1408374827 2014-08-18 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well was completed in 09/04/2007. The unit is producing and production reports are current. The producing unit is accessed via a dedicated, gravel-paved access road which extends north from the terminus of the current residential driveway. The access road is level, and well drained with no signs of erosional deficits. Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger-lift system and aboveground storage tank (AST) battery in board-on-board enclosures. The wellhead and surrounding area are free of signs of leaks or other impacts. Wellhead fittings and valve structure is intact and shows no signs of leaks or problems with corrosion or integrity. The battery is comprised of 1 steel 100-barrel production tank, 1 100-barrel steel brine tank, and separator unit. Spacing from well to battery, and spacing of elements within the battery meet regulations. The load-outs are bull-plugged. There is secondary containment at the battery and the interior areas are dry. I did not observe any signs or evidence of leakage, spills, or problems. Unit identification placards are present at both the battery and wellhead and are in good condition. No issues or problems requiring attention at the time of this inspection.

1476457840 2016-10-14 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued Compliance Notice. At the time of the previous inspection, I had noted that the tank lids were not capable of sealing as required by OAC 1501:9-9-05(E)(4)(d). At the time of this inspection, the lids had been replaced and were in compliance. I inspected the rest of the production elements and observed the following: fittings and connections at the wellhead and battery were secure and free from leakage or problems. The secondary containment was intact and the interior was dry. The enclosure gates were locked and secure. The shared access road was in good condition.

At the tiem of this inspection, I observed the following: The wellhead and tank battery enclosures were locked and secure. Tank lids were closed and sealed. Load lines were closed, plugged, and secured with locks. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I walked the areas surrounding the wellhead and battery which are comprised of meadow grasses and lightly wooded areas, and found no evidence of problems or impact to the surrounding greenscape attributable to the on-going production activity.

The well and associated tank battery are located in the back of a single-family residence. At the time of this inspection, the enclosure gates at the wellhead and battery were locked and secure. Fittings and connections at the wellhead were secure and free from signs of leakage or problems. Areas around the wellhead and battery were comprised of grassy vegetation and wooded areas. I inspected the areas surrounding the wellhead and battery and did not observe any signs of problems or impacts. I observed the following maintenance deficiency: The production tank lids were old style hatch lids which do not seal and are not compliant with OAC 1501:9-9-05(E)(4)(d). The hatch lids must be replaced with lids with functioning seals. This safety deficiency must be addressed by 01/15/2016.
