Elements of the well consist of a production wellhead with standard valve assembly and mechanical plunger lift system. The wellhead is located in a closed and locked board-on-board enclosure. There are no signs of leakage or problems at the wellhead. The aboveground storage tank battery is located west and adjacent to the wellhead. The battery is comprised of 2 steel 100-barrel production tanks, and a horizontal separator. The battery is located in a separate board-on-board enclosure which is gated and locked. The load-outs are bull-plugged and locked. There is process safety equipment in use (carbon filter, low-pressure relief valve, and spark arrestor). The secondary containment at the battery appears well maintained. I did not observe any signs or evidence of leakage, spills, or problems. There is ID signage at the battery and wellhead which are legible and in good condition. The area of the well is generally level with a slight topographic drop to the west. Surface draining appears to be controlled by local infiltration. I did not observe any signs of gully formation or washouts. Access to the unit is afforded by a gravel-paved access road which is partially shared with the house at 29750 Havard Rd. The short access road is currently in good condition and free of washout or erosional deficits. Beyond the area cleared for the wellhead and tank battery, the areas surrounding the unit are heavily wooded with one single-family residential home bordering to the south, and no other residential strucutres within eyesight. |

The well and associated tank battery are located in the back of a single-family residence. At the time of this inspection, the enclosure gates at the wellhead and battery were locked and secure. Fittings and connections at the wellhead were secure and free from signs of leakage or problems. Areas around the wellhead and battery were comprised of grassy vegetation and wooded areas. I inspected the surrounding areas and did not observe any evidence of problems or impact. I observed the following maintenance deficiency: A portion of the battery enclosure fence is broken down and requires repair/replacement. I also observed the following safety deficiency: The production tank lids were old style hatch lids which do not seal and are not compliant with OAC 1501:9-9-05(E)(4)(d). Hatch lids must be replaced with lids that have functioning seals and must be closed and secure . These safety deficiencies must be addressed by 01/16/2016.
