0147740352 | 2013-01-08 | UrbanProduction Wells | No Violations | The producing unit is comprised of a two-tank battery (2 140-barrel, 1 20-barrel) with mechanical separator and wellhead valve assembly. The tank battery is enclosed in an 8-foot enclosure. A mechanical plunger lift is affixed to the wellhead. The well is currently producting. The secondary containment is adequate. Unit identification is present and legible at the tank battery. I did not observe any leaks, spills, or other signs of contamination. |

0756568130 | 2018-05-30 | UrbanProduction Wells | No Violations | I inspected this well location as part of routine inspection activity. At the time of this inspection, the enclosure gates were locked. The load line valves were secure. The tank lids were closed. The well was in active production with a current chart record. The secondary containment was intact. Fittings and connections at the wellhead and battery were secure. The surrounding areas are comprised of paved parking and drive areas. I did not observe any signs of problems. |

1135489379 | 2017-08-11 | UrbanProduction Wells | No Violations | I inspected this well as a follow-up to a previous inspection in which I had noted that the former area of the pits had become severely heaved, causing significant, large longitudinal cracks in the over-lying asphalt paving, rending this portion of the parking lot unusable. I had notified the owner about the observed condition. At the time of this inspection, work to remedy the heaving was on-going, and expected to be completed soon. |

1364400598 | 2017-08-16 | UrbanProduction Wells | No Violations | I inspected this well location as part of routine inspection activity and a follow-up to a previous inspection in which I had noted that the area of the former pit was showing some signs of heaving. Specifically, the asphalt paving had become heaved up by several inches along an east to west trend, co-incident with the former location of the drilling pit. I had notified Bass Energy about the heaving and measures to correct the heaving had been undertaken.. At the time of this inspection, I observed that the heaved area had been effectively remedied, and was level with the surrounding area. I also inspected the production elements. The enclosure gates were locked. The load line valves were secure. The secondary containment was intact. Fittings and connections at the wellhead and battery were secure. The surrounding areas are comprised of paved parking and drive areas. I did not observe any signs of problems. |
1489430500 | 2017-03-13 | UrbanProduction Wells | No Violations | I inspected this producing well as part of routine inspection. At the time of this inspection I observed the following: a wellhead with plunger lift system and associated tank battery in separate board-on-board enclosures which were locked and secure. The valves and connections at both the well and battery were secure and free from signs of leakage. Process safety equipment was present. ID signage was present and accurate. The areas surrounding the well and battery were comprised of asphalt parking and drive areas for the hotel facility located on the parcel. I walked the surrounding areas and found no signs of impacts or degradation. Access to the well and battery was afforded by shared-use, asphalt-paved driveway which was in good condition. I noted that the asphalt paving near the wellhead was heaved up along a more-or-less east-west axis. I notified Ralph Gregory about the heaving. I advised that it should be evaluated to assess whether the heaving is related to the drilling or production activity. |
