0591628478 2012-08-28 UrbanProduction Wells No Violations On 8/28/2012, I completed an urban production well inspection status check. My inspection found the well equipped with a plunger lift unit (rabbit) that was leaking brine and oil. The contaminated area was approximately 3 in diameter. I called the well tenders, Strawn Oil Service, at 330-537-3863 and asked for the leak to be cleaned up by 9/3/2012. The tank battery contained one 100 barrel oil storage tank, one 50 barrel brine storage tank, a separator, and the proper identification posted. The lease road was in good condition.

The well was equipped with a plunger lift assembly. A domestic gas line came off of the 4.5 casing. The tank battery consisted of a 100 barrel production tank, a 25 barrel, poly, brine tank and a mechanical separator. A dike surrounded the tank battery. Identification signs were posted. There was a plug on the load line. The lease road was in stable condition. The well was placed into production prior to House Bill 278.
1346846986 2012-09-05 UrbanProduction Wells No Violations I completed a follow-up NOV inspection for an oil/brine leak around the well head. My inspection found the contaminated soil removed and clean top soil spread around the well. Inspection passed.
