0049451011 | 2013-03-26 | Production Wells | No Violations | Follow up to -276953186. The tank has been emptied and is no longer leaking brine. The contaminated area around and below the tank still has not been remediated. |
0207223029 | 2014-04-30 | Production Wells | No Violations | Follow up to -276953186 and 1312821519. The well is overgrown with vegetation. The valves on the production is closed but the valve on the tubing is open. Work has been done around the old leaking tank, but it still remains on site and there is still contaminated soils. I spoke with Mike Benidum of Black Rock and he states a new meter will be put on the well sometime next week and the tank should be removed after that. |
0246545987 | 2013-12-24 | Production Wells | No Violations | Follow up to -276953186. The tank does not appear to be leaking brine, but the contaninated soils remain on site. |
0276953186 | 2013-02-19 | Production Wells | Violations Noted | The plug at the end of the load line on the bottom of the 100 barrel (Bbl) steel tank was leaking a small amount of fluids and contaminating an area below the tank roughly 15 x 4. The fluid dripping from the valve was field tested for the presence of salt with a Quan-tab and found to have 6508+ ppm Chlorides. There was no dike around the tank and the tank was situated roughly 20 feet from the county road and in a way so that a potential spill may affect the traveled portion of the road. All contaminated soils shall be removed and disposed of properly. The affected area shall be seeded and mulched and steps shall be taken to encourage vegetative cover. Plug in load line shall be repaired or replaced. Tank(s) shall be placed inside a dike with a capacity enough to hold 110% of the largest vessel and meeting all current spacing requirements. |
0330576174 | 2011-10-28 | Administrative Inspection | No Violations | Spoke with Mr. Richman of Blackrock Energy. He let me know they would be reconstructing the Tank Battery site. |
0436979919 | 2019-04-22 | Production Wells | No Violations | My inspection of this well showed the well is idle and incapable of production. The last reported production was in 2008. The well was originally cited for pollution and contamination. The site has self remediated itself. The well lacked legible identification. Nothing has been done to bring the well into compliance. The owner of the well shall place legible identification at the well or at the tank. |
0486300629 | 2012-03-19 | Administrative Inspection | No Violations | Spoke with David Richman of Blackrock regarding plans for bringing the well into compliance. 304.782.4350. |
0677247357 | 2016-06-06 | Production Wells | No Violations | The well and the open violations remain in the same state on this inspection. I called Angus Energy and the gentleman said he would pass my call on. I am awaiting the return call. |
0691049187 | 2016-01-19 | Production Wells | No Violations | Follow up to -276953186. I spoke with david richman at Angus energy about cleaning up the tank. |
0795520791 | 2014-06-03 | Production Wells | No Violations | Follow up to -276953186 and 1312821519. Valve on tubing open and valve on production closed. No identification at well or tank. Old tank still on site and no meter at well. |
0966277024 | 2018-04-16 | Production Wells | No Violations | This well is still idle and the old tank is still sitting in the same location. The brine scald is no longer visible. The tank battery consists of 1-100 barrel steel tank and a vertical separator. The tank battery has no dike around it and is about 9 feet from the township road. The well is equipped with a pumping unit, tubing and rods. The area around the well is very overgrown with multiflora roses and other vegetating so much so that getting to the casing head was virtually imposible. I spoke with Brett Bendimen last month about this well. |
1093320347 | 2015-12-03 | Production Wells | No Violations | Follow up to -276953186. Scald area remains below tank alongside road. |
1141045705 | 2015-08-04 | Production Wells | No Violations | I went to the well location to take pictures for Inspector Josh Garland. |
1174757690 | 2018-12-03 | Production Wells | No Violations | The well was originally cited a violation for pollution and contamination and to be plugged. The bottom of the tank got a hole in it and drained down the side of the bank into the road ditch. The pollution has since remediated itself. The well is still idle and not in production. The well is equipped with a pumping unit, tubing, rods and a Fairbanks engine. The area around the pumping unit has a fence around it and is very overgrown with briars, weeds, vines and brush. The tank battery consists of 1-100 barrel steel tank with a hole in it and a vertical separator. I spoke with Bret Bendum about this violation and others on Friday, the 30th of November. |
1244204729 | 2004-07-23 | Production Wells | No Violations | No production records. |
1296958269 | 2016-06-07 | Administrative Inspection | No Violations | Talked to Bret who is going to meet me at the Flanders well tomorrow at noon. Landowner will not let him have access to the well. |

Well was not properly identified, no production records have been filled with DMRM since 2008, production tank had leaky valve which had allowed brine to escape. Brine scalded area was 4x15 feet and measured 6797+ ppm chlorides with a quantab. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number. Valve on production tank shall be replaced or repaired. Contaminated soils shall be removed, reseeded and mulched. Well shall be produced, plugged, or placed in temporary inactive status. |
