0083672688 2017-08-10 Production Wells No Violations I stopped by this well today to do a field inspection on it. I noticed the new tanks have been set. A few months ago the flow line busted and the company talked about moving the tanks and flow line so this does not happen again. At this time 2-100 barrel tanks have been set. A new dike has been put around the production tanks and the flow line has been hooked up. The identification for the well is located on the tank. There are no violations at this time.
0359791844 2017-04-03 Production Wells No Violations I returned to this well today to check on the cleanup of the contamination from the flow line leaks. At this time the cleanup is almost complete. The crew ended up working through the weekend to get the job done. They should finish cleaning up the rest or the contamination today. The old flow lines have been cut up and stacked up to be hauled off of the location. The company will be replacing the flow line and rerouting it. They met the landowner Mr. Francis over the weekend and he showed them where he would like the line. The company will burry the line at the edge of the field. I will check back as needed until the job is finished.

0727841032 2015-12-16 Production Wells No Violations I was on location to do a field inspection on this well. The well has a pumping unit on it with rods and tubing in it. The valves are open from the wellhead to the production tank. The identification for this well is located at the tank pad. There are no violations at this time.
1139696158 2017-03-26 Production Wells Violations Noted I responded to a complaint that went to the duty officer in the Emergency Response section. I went to the site of the complaint and met with the landowners. I first inspected the tank battery and found one of the steel tanks heater tube was leaking. The landowners then escorted me to the strip pit/pond that oil was floating on. This was behind the tank battery about 75 yards. There were three plastic lines going through a narrow point in the pond. Two of these had repairs made to them. The 2 inch plastic line had band clamps on it. The 1 1/4 inch plastic line had a union and worm gear clamps. The landowners then took me to the well where the lines were coming from.. On the way back from the well it was discovered that a flow line had leaked and an area of 30 feet in diameter was affected with oil. I spoke with a representative from the company who came out on location and went over everything that will need completed. The contaminated soil and brush will need removed from the leaking flow line area. This dirt will need disposed of in the proper manner and the area will need seeded and mulched. The heater tube that is leaking shall be fixed. The oil that is on the pond and the brush and soil affected shall be removed and disposed of in the proper manner. The electric power supply line will need to be properly hung as to make it safe per-Electrical Codes

2. 12 Foot Overhead Clearances For Conductors And Cables Not Over 600 Volts In instances over residential property and driveways, and those commercial areas not subject to truck traffic where the voltage does not exceed 300 volts to ground, should be a minimum of 12 feet overhead clearance. 3. 15 Foot Overhead Clearances For Conductors And Cables Not Over 600 Volts However, in the 12-foot classification areas that exceed 300 volts, these cables should have a minimum clearance of 15 feet above grade.


1333559801 2017-04-10 Production Wells No Violations I returned to this well today to check on the status of it. Everything appears to be cleaned up. I do not see any signs of oil on the new pads that were put down. I will let the company know they can remove the pads and boomers from the pond. Once that is done I will pass the compliance notice.

1490790286 2017-03-29 Production Wells No Violations I was on location today to do a follow-up on the cleanup of this well where the flow line had busted in two different areas. The company was not on location while I was there in the morning doing my inspection but they did call to let me know they had arrived after I left. They have been using different absorbing materials to soak up the oil from the pond and the ground. At this time they have been able to contain the contamination from spreading. They will continue to work on it every day until it is cleaned up. I will return to the location tomorrow to check on it.
1491479086 2017-04-06 Production Wells No Violations I returned to this well today to do a follow-up inspection on it. At the time of my inspection no one was on location. All of the contaminated soil has been bagged up and is stored in the trailer for removal. There are pads put down at this time to soak up anything else that might come up. The company will be monitoring the area for any more contamination the next couple of weeks.
1493121747 2017-04-25 Production Wells No Violations I returned to this well to do a follow-up inspection on it. During my inspection I found the clean-up materials still on location so I contacted Joe Schmeltzer to see what was going on with it. I did not reach him at the time so I left him a message. Joe returned my phone call later in the day and he told me the pumper was waiting on me to tell him it was okay to pick up. I let Joe know that I had already done that a few weeks ago. He said they must of misunderstood and would get it taken care of right away. I will return in a couple of days to check it.

1498312469 2015-01-15 Production Wells Violations Noted During the field inspection of this well I found contamination around the wellhead. The stuffing box rubbers are leaking causing oil to displace around the well. The contamination will need to be cleaned up and removed from the location.
1792732912 2015-05-13 Production Wells No Violations I was at this well to do a follow-up inspection on it. The violation has been resolved and the well is in-compliance. There are no violations at this time.

At this time they have been able to remove most of the contamination from the site and they are working on rerouting and replacing the flow line today. I will continue to inspect the location until the job is finished.
