0242849306 | 2016-05-10 | Production Wells | No Violations | This inspection is a follow up inspection to the NOVs 1310396632, issued on 2/28/2012. I found an 8 5/8 X 4 1/2 and a 4 1/2 X 1 1/2 wellhead, operated by a plunger lift system. The valves are in the open position. One 210 bbl. tank in an earthen dike. A production separator is on location. The tank is properly identified. The last production was in the year of 2013, and the last year of reported production was 2013. |
0821025423 | 2014-05-01 | Production Wells | No Violations | On the day of my inspection following up on NOV-1310396632, I witnessed that the production equipment included a 210bbl tank within an earthen dike and a horizontal separator. The valve was closed on the sales line. A 2 inch sales meter with a .75 inch orifice was located south of the production equipment. The well was equipped with 8.625 inch casing; 4.5 inch casing, 1.5 inch tubing and a plunger lift system with the valves open on the tubing and the valves closed. Production records were current through 2012 and identification was present. I made call to Blaze Oil & Gas at 330-345-6700 and spoke with Dave Sohl and asked him what the plans for the well were and he stated that it was just shut in a couple of weeks ago due to a hole in the sales line. He stated it would be repaired and back online in a week and would let me know so I could do a follow up inspection. |

1632671844 | 2017-08-28 | Production Wells | No Violations | Walked with Inspector Akins to this well as it was part of an H2S complaint. A faint rotton egg smell near the well but no leak was heard or found but the 8-5/8 x 4-1/2 head was under ground. The H2S monitor did not activate. This well is producing. |
