1277132178 2010-06-01 Production Wells No Violations arrived on location and met with rocky grimm and joe cheslock I made an inspection of buried saltwater tank and found know violations , grimm advised the company was going to remove the tank anyway and that mr cheslock will contact me when they are ready.
1427375093 2018-09-21 Production Wells Violations Noted Upon my arrival on location, I witnessed the well was equipped with 4.5 inch diameter casing, 2.375 inch diameter tubing, rods, and a pumping unit ran with an electric motor. The well was shut in at the time of the inspection. The well had a five-foot diameter circle of oil around it. The oil was contained but needs to be cleaned up. The production equipment included a steel 100 barrel tank, a concrete vault, and a horizonal separator within a concrete containment. The production equipment and the well had a chain link fence around them. The production report was not up to date and there was no identification at the well. The Howden Company needs to submit a written plugging schedule to Inspector Jeffrey Adams at 2207 Reiser Ave. New Philadelphia, Ohio, 44663 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The owner/operator will be notified if the plugging schedule is approved or denied.
1438175642 2015-07-29 Production Wells No Violations I met Patrick Flemming with Howden on site. The issues with the tank battery containment has been taken care of. The well is idle. I explain to Patrick, if Howden was not going to use the well, they need to start making plans to plug the well.
