The pit liners are in good shapeand there is a tarp over blow pit to contain dust. There is a 300 barrel brine in front pit and a gas detector in use. No leaks or spills around rig. Permit displayed in doghouse and at location entrance. No violations at this time.
0711629458 2017-12-13 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-3168. Inspector John Fleming was present during the inspection. A review of RBDMS indicated production reporting was current. Access to the well was through a residential lot. The well head and tank battery were enclosed by a locked chain-link fence enclosure. Correct identification was present. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No leaks were observed. All production equipment was within a locked chain-link fence enclosure. Correct identification was present. A stable earthen dike that was free of fluid was present around the equipment. Equipment within the dike consisted of one (1) horizontal separator, one (1) 100 bbl production tank and one (1) 50 bbl poly brine tank. The separator remained in good repair and contained a current 7 day production chart. The production tank remained in good repair. Valves were closed, locked and plugged. A charcoal canister and lightening arrestor were present. The poly brine tank remained in good repair. The inlet valve was closed and locked. No violations were observed during this inspection.
0951126633 2012-01-18 Preliminary Restoration No Violations On the day of my inspection I completed a status check on the pPeliminary Restoration of this site and found that the tank battery dike was constructed. The tank battery equipment was in place and consisted of one new 110 barrel stock tank, one new 30 barrel poly brine tank and one new separator. The well head was equipped with a plunger lift system. The fencing around the well head and tank battery was not yet in place, the entire location was still fenced with chain link. The site was rough graded, with one soil stockpile remaining. This well is not yet in production, there are no violations and the site does not pass Preliminary Restoration at this time
1061745363 2012-05-11 UrbanPreliminary Restoration No Violations I conducted a status check on the restoration of this site and found that all drilling equipment and debris has been removed, the soil has been graded out to the approximate original contour and good vegetative growth has been established. The well was equipped with a plunger lift system, and fenced with chain link and three-strand barbed wire. The fence was locked and proper identification was posted. The tank battery consisted of one 100 barrel stock tank, one 50 barrel poly brine storage tank, one separator and an earthen dike. The stock tank was equipped with a flash arrestor, charcoal filter and bull plug. The tank battery was fenced with chain link and three-strand barbed wire. The fence was locked and proper identification was posted. I found no leaks or spills. No violations at this time. This site passes Preliminary Restoration.

The tank battery consists of one 100 barrel stock tank equipped with flash arrestor and charcoal filter, one 50 barrel poly brine storage tank, and a separator. The dike was in place and the tank battery was fenced with chain link, and three-strand barbed wire. The entire location is fenced with chain link, and identification was posted at the site entrance. The location still needs to have top soil stockpile graded out and seed/sod. This site does not pass preliminary restoration at this time.
1320413667 2011-11-01 Urban Drill / Deepen / Reopen No Violations Wildcat Drilling Rig #1 drilling surface hole on fluid, at depth of 746. Chain link fence installed around entire location, silt fence installed along north edge of location, topsoil stockpiled around pits as berms, permit posted at location entrance and in doghouse. Pit liners in good condition, no leaks or spills. No violations at this time.
1372410985 2011-12-06 Preliminary Restoration No Violations The entire location is fenced with chain link and there ia a permit and identification (ID) sign posted at location entrance. The trash/debris on location still needs removed and the site needs graded and seeded. The tank battery has not been constructed. No violations at this time.
