0815094771 | 2018-09-11 | Plug / Plug Back | No Violations | I was on location today to witness the plugging of this well. Barry Bowers has moved his service rig in on the well to plug it. They have already pulled the rods and tubing out of the well. There is a packer ran in the well on 2-7/8 diameter tubing. Barry had JT Wireline services come out to log the well so we could locate the packer. Once the packer was located the tubing was shot off right above it so we could set the bottom plug through the 2-7/8 inch tubing. The bottom plug was pumped and then the rig crew pulled the tubing up to 2258 feet. The pipe was dragging all the way up to this point. We believe the 4.5 inch casing has collapsed onto the tubing. JT wireline services ran his gun back down the well to perforate the Lime so we can pump the Big Lime plug. Cemenco then set a 200 foot Big Lime plug before the tubing was pulled the rest of the way out of the hole. |
0973443128 | 2011-07-27 | Annular Disposal | No Violations | Follow-up on Compliance Notice, In-Compliance. Company has well in production, company is planning on selling well to landowner. |
0976981212 | 2014-01-23 | Production Wells | No Violations | This well appears to be in production. The well has a pumping unit on it with rods and 1.5 tubing. There is also 2-7/8 in the well. The well appears to have a packet ran in it. The production valves are open to the tank pad at this time. The identification for the well is located at the tank. There are no violations at this time. |
1044622077 | 2017-06-19 | Production Wells | No Violations | I was on location today to do a field inspection on this domestic well. The well appears to be hooked up and in use at this time. There are no violations to report on this well. |
1139264505 | 2015-10-20 | Administrative Inspection | No Violations | Mr. Emmert called me about 7:15 last night. He stated he was waiting for the corn field to be harvested and then will remove the tank. I asked how long before the corn is harvested and he said no more than two weeks he would suppose. He said he was going to use his tractor to remove the tank. I also informed him that the API number on the tank was wrong. He said to leave him a voicemail with the correct number and he would change it on the tank. |

1427977607 | 2015-04-02 | Production Wells | Violations Noted | Inspection on 4/2/2015 revealed well is capable of producing. Tank battery consist of two 100 barrel tanks. Identification was not legible at tank battery. Upon further inspection of the tanks, one tank was found to be leaking with a piece of wood wedged into the heater tube. There was oily material inside the dike. Oily material was also found around the well head. Full and legible identification is needed at the well head or tank battery. Repair or replace leaking tank. Clean up and remove all oily material from the dike area and from around the well head and dispose of in a proper manner. |
1431448983 | 2015-05-08 | Administrative Inspection | No Violations | Spoke with the owner of the well James Emmert who stated that he just finished the identification on the tank to comply with notice. 1537282641 | 2018-09-18 | Plug / Plug Back | No Violations | I am back on location today to witness the plugging of this well. Today the 1.5 inch diameter tubing was ran back in the well to set the Berea plug. The tubing was then pulled up to the bottom of the surface so they could set a 200 foot plug before working to retrieve the 8 inch casing. Once the rig crew got the 8 inch casing free they pulled up and found out the 10 inch that was left in the well was only 3 feet long. The records showed there was 65 feet of 10 inch in the hole. The rig crew had to put the 8 inch back in the well to protect the fresh water zone. The 1.5 inch tubing was then ran back in the well down to 200 feet so they could pump the surface plug. They pumped 60 sacks of cement down the well then shut down to let it setup over night. They will tag the plug in the morning and go from there. |
1548942143 | 2019-01-31 | Final Restoration | No Violations | I was on location today to do a final restoration on this well. At this time I can not complete the restoration because AEP is building a sub station on location. They wanted to leave the casing exposed until they were finished for a landmark. I contacted Barry Bowers while I was on location and he is going to contact AEP to see if he can cut it off yet. |
1696125911 | 2018-09-07 | Plug / Plug Back | No Violations | Barry Bowers has moved his service rig in on this well to plug it. They are planning on pulling the rods from the well today and get ready to set the bottom plug on Monday. |
1749050199 | 2011-06-14 | Production Wells | Violations Noted | An inspection of the well revealed well is capable of producing, no production has been reported since 2006 on well. Must report production on well or place into temporary inactive status. ID not visible do to weeds and brush growing over ID at tanks. Need full and legible ID at the well head and tank battery. |
1869060544 | 2018-09-19 | Production Wells | No Violations | I was back on location today to cover the plugging of this well. Today we ran the tools down the well to tag the surface plug. It has slid own to 196 feet so we ran some 1.5 inch tubing back in the well to top the well off. The cement plug was pumped to the surface and it stayed at the surface. The well is now plugged. |
