| 0036175691 | 2010-05-19 | Production Wells | Violations Noted | On site with Jim Smith. Jim dug up the 8 5/8 pipe, leak appeared to be coming from around the ring at the 4.5 inch pipe and annulus. The annulus is tied into the flow line, the flow line valve was closed. Took union after the valve apart, put an ODNR DMRM needle gauge on the line, annulus pressure was 10 psi. Operator opened valves, went to tank battery, opened valve to the tank battery, flow line at the tank battery had a leak in it. Per the operator, there is a leak in the 4.5 inch pipe. 4.5 inch pipe needs repaired or well must be plugged. Tank battery, all line leaks need repaired. There is a cement under ground tank, south east corner of the battery, the tank is no longer in use. The tank is partially submerged. Tank needs removed. There is a depression on the east side of the dike, that is allowing water out of the dike. Tank battery dike needs to be sufficient to prevent the escape of brine and oil. Two wells are produced into one tank battery, each well needs its own identification sign. Tank battery needs to be kept free of water and vegetation. |
