0383073185 2018-05-25 Final Restoration No Violations Upon arrival on location to do final restoration inspection, I witnessed miscellaneous pipe and fittings laying on site. The location had not been reclaimed or seeded. Made a phone call to well owner about this situation.
0883572143 2015-04-06 Production Wells No Violations On the date of inspection I found the well to be equipped with a gas plunger lift, 1.5 inch tubing, 4.5 inch casing, and 8.625 inch casing. At the time of my inspection all valves on the well head are in closed position. I was unable to locate an associated tank battery. There is no identification on the well. According to RBDMS, the well has not produced since 1995.
1366648622 2017-12-20 Plug / Plug Back No Violations Bishop Well Services lowered their ripping tools into the 4.5 casing to determine the top of the reset Clinton plug. The fluid level was found at 670. The top of the reset cement plug was found at 4129, approximately 411 feet above the top perforation at 4540. The Clinton plug was compliant with OAC 1501:9-11-08 - (A). I gave the Operator the ok to proceed with the plugging of the well. Bishop parted the casing at 3160 and recovered 75 joints and one (1) pup joint of 4.5 casing. Plugging operations ceased pending Petroset Cementing Services availability. The well was secured overnight.
1513705432 2017-12-19 Production Wells No Violations
1784994048 2017-12-18 Plug / Plug Back No Violations I was on location to witness the plugging of the Krisman Enterprises # 3. Bishop Well Service (BWS) was the plugging contractor with Petroset Cementing Services supplying the cement and pumping services. Appalachian Energy Services (AES) was the water hauler ( both fresh and waste ) and supplied excavating services. The well was opened ( both 4.5 and 8.625 casings ) and was static after exhibiting a puff of gas. Bishop Well Services removed the 1.5 tubing to assess its suitability for use as a work string. Total tally recovered was 4585. Fluid level was noticed, on the tubing, at 2300. After removing the mud anchor and adding two (2) joints of 1.5 tubing, Bishop Well Services lowered the 1.5 work string to 4640. An in ground pit ( 20 X 20 ) was dug northwest of the wellhead and lined. The mix water parameters were 33 F and pH was 7. Petroset Cementing Services pumped 40 barrels of fresh water in order to establish circulation. Petroset mixed 7.3 barrels ( 30 sacks of Class A blended with 2 % gel ) of cement slurry ( cement density was verified, by scale, at 15.1 ppg ) and displaced through the interval 4640 to 4225. Bishop removed the 1.5 work string from the 4.5 casing, fluid was noticed at 80, and shut in the well overnight. No violations identified during this inspection.
1835474671 2018-01-04 Plug / Plug Back No Violations On the day of my inspection I found that the 8 5/8 inch diameter casing has not been cut off yet. There are no signs on bubbling.
1901058815 2017-12-19 Plug / Plug Back No Violations Bishop Well Service ( BWS) was the plugging contractor for the well. After setting the Clinton plug on the previous day, BWS lowered their ripping tool in order to tag the top of the plug. Fluid level in the 4.5 casing was indicated at 2300. The top of the Clinton plug was found to be at approximately 4580. The perforated interval was from 4624 to 4540. The Clinton plug was not compliant with 1501:9-11-08-(A) and I informed the Operator that the plug would have to be reset. Bishop removed the ripping tools from the 4.5 casing and began to lower the 1.5 work string to re-spot the Clinton plug.
